摘 要:目的检测宿主血清中旋毛虫循环抗原 ( CAg)以诊断本病和考核药物疗效。方法双抗体夹心 -ELISA法。结果 94只实验感染旋毛虫小鼠 ,受染 3d后 ,即有 7%的小鼠显示阳性反应 ,受染 30 d,无论受染幼虫数目多寡 ( 50、1 50及 30 0条幼虫 /只 ) ,全部受染鼠 ( 1 0 0 % )均显示阳性反应。给予丙硫咪唑治疗后的第 1周 ,CAg的阳性率仍为 1 0 0 % ,但在治疗后第 2、3及 4周 ,则分别降为 60 %、2 0 %及 1 0 %。检测 61头感染旋毛虫猪血清中 CAg,4 0头 ( 65.6%± 6.1 % )呈阳性反应。 1 0 0头正常猪均为阴性 ,30头感染囊尾蚴和 30头感染弓形虫的猪 ,仅 1头感染弓形虫的猪出现轻度交叉反应。检测 36例旋毛虫病人血清中 CAg,2 6例 ( 72 .2 % )呈阳性反应。 50例正常人皆为阴性。 1 4 2例其他 9种寄生虫病人 ,仅 1例囊尾蚴病人出现轻度交叉反应。结论旋毛虫 CAg的检测具有早期诊断和考核药物疗效的价值。Aim To detect the circulating antigen(CAg) in sera from experimentally infected animals and patients with trichinosis for early diagnosis and evaluation of treatment efficacy Methods Double antibody sandwich-ELISA Results The CAg in sera from 94 mice examined at different periods after infection revealed positive (7%) 3 days after inoculation and it became strongly positive in all animals 30 days after inoculation no matter how many (50, 150 or 300 larvae/mouse) T spiralis larvae were inoculated The CAg positive rate remained 100% 1 week after treatment with albendazole while it decreased to 60%, 20% and 10% 2, 3 and 4 weeks after treatment, but the specific antibody titers remained in high level Sera of 61 infected pigs showed CAg positive rate of 65 6% while all 100 control normal pigs were negative In another group of 60 swine sera with other infections, only one with toxoplasmosis showed a weak cross reaction The positive rates of CAg with mono- or poly-antibodies of sera from 36 trichinosis patients were 67 7% and 72 2% respectively, while sera from 50 normal persons gave negative result 142 sera from patients with various other parasitic infections i e paragonimiasis, schistosomiasis, clonorchiasis, ancylostomiasis, filariasis, cysticercosis, echinococcosis, hepatic amoebasis and giardiasis showed a cross reaction of only 0 7% Conclusion The results have indicated that CAg detection is useful in the early diagnosis of trichinosis and evaluation of treatment efficacy
