摘 要:目的从患者角度了解公立医院的公益性状况,并分析不同的三级医院在公益性方面的情况差异,为提高公立医院公益性提出政策建议。方法对公立医院就诊患者进行现场调查,分析患者对医院公益性的评价情况;通过聚类分析,将纳入调查的公立医院划分为不同类别;通过F检验,分析不同类别公立医院的各公益性指标得分情况。结果通过聚类分析,将纳入研究的17所三级医院划分为四类;各类三级医院的12个公益性指标得分差异均有统计学意义,得分在1.12~2.70之间。结论发挥政府主导作用,三级医院承担相应的惠民公益服务,并从控制费用、合理检查等方面增强服务适宜性,将有助于提高其公益性。Objective To compare the differences in public welfare among tertiary public hospitals from patients" perspective and provide policy recommendations to improve the social responsibility of public hospitals. Methods Field survey data were collected in public hospitals. The patients" assessment (12 items) on public welfare of public hospitals was analyzed using a hierarchical cluster analysis to classify the tertiary public hospitals into different categories. F-test was applied to detect the differences of public welfare among the hospitals by category. Results According to the hierarchical cluster analysis, the sample of tertiary public hospitals was divided into four different categories. The differences in each patient-reported item of public welfare among the categories were statistically significant. Conclusion Under guidance of the government, tertiary hospitals are expected to provide due public benefits, and enhance service fitness in terms of expense control and justifiable medical examinations.
