摘　　要：目的 探讨改良Sugiura手术对门静脉高压患者肝脏血流的影响.方法 将106例门静脉高压症患者随机分为两组,对照组40例,观察组66例.对照组采用单纯行贲门周围血管离断术治疗,观察组采用改良Sug-iura手术治疗,术后1个月观察患者肝脏血流的变化.结果 两组手术后肝总血流量（THF）、门静脉血流量（PVF）和肝动脉血流（HAF）均有明显改善,并且观察组比对照组改善更明显（P＜0.05）.采用Child分级评价肝功能,两组在术后肝功能分级均有明显改善,并且观察组比对照组改善更明显（P＜0.05）.结论 改良Sugiura手术可以更明显改善门静脉高压症患者肝脏血流,从而改善肝脏功能.Objective To investigate the effect of modified Sugiura surgery on hepatic blood flow in patients with portal hypertension.Methods One hundred and six patients with portal hypertension were randomly divided into two groups,the control group （n =40） and the observation group （n =66）.Patients in the control group were operated through simple line pericardial blood vessel disarticulation,while patients in the observation group were operated through modified Sugiura surgery.The hepatic blood flow were compared before and after one months of operation.Results The total hepatic blood flow,portal vein blood flow and hepatic arterial flow improved after one month of operation.There were significant differences between the two groups （P ＜ 0.05）.According to Child grading,the liver functions in both groups improved after operation,there was significant difference between the two groups （P ＜ 0.05）.Conclusions Modified Sugiura surgery can improve hepatic blood flow in patients with portal hypertension,so as to improve liver function.
