氨氯地平对高血压病患者血浆内皮素及胰岛素敏感性的影响

摘　　要：目的 :比较氨氯地平、硫甲丙脯酸及硝苯地平对高血压病患者治疗后血浆内皮素、胰岛素水平变化以及对胰岛素敏感性的影响。结果 :三种药物均可使高血压病人的血压降低 ,治疗前后比较 (P <0 .0 1) ,血浆内皮素水平亦有不同程度的降低 (P <0 .0 1和P <0 .0 5 )。并且氨氯地平和巯甲丙脯酸一样 ,治疗后可使血清胰岛素敏感性明显升高 ,与治疗前比较 ,P <0 .0 5。结论 :氨氯地平及巯甲丙脯酸在降压作用的同时还可改善胰岛素抗性 ,且氨氯地平可缓解冠脉痉挛 ,又具有长效、缓释、血药浓度稳定、无肾脏毒副作用的特点 ,在高血压病的最终防治目标中更具有较为理想的效果。WT5”BZ]Purpose:Amlodipine,Captopril and Nifedipine were used to treat respectively 72 patients with essential hypertension(EH).In order to observe the changes of plasma endothelin(ET) and insulin(IS) levels and that effect on insulin sensitivity.Result:The blood pressure significantly were reduced in every drug of all three after treatment than before treatment(P<0.01),and the ET levels were drooped after treatmet than before freatment(P<0.01 P<0.05).And the Amlodipine yet can droop plasma Is Ievels and increase the insulin sensitivity as the Captopril can after treatment than before treatment(P<0.05).Conclusion:Insulin resistance were improved when the blood pressure of patients with EH were reduced by Amlodipine and Captopril.Amlodipine has characteristics of relaxing coronary spasm,long-acting,slow released,stable blood concentration and no side-effect in the kidney.therefore,Amlodipine has better efficacy in aim on treatment of EH.
