摘 要:目的探讨星状神经节阻滞对老龄大鼠血清S100β、NSE及术后认知功能的影响。方法健康雄性SD大鼠48只,18-20月龄,体重500-700g,采用随机数字表法分为:对照组(C组)、星状神经节阻滞组(S组)和生理盐水组(N组),每组16只。C组解剖暴露右侧颈交感神经干但不阻滞;S组解剖暴露右侧颈交感神经干并用0.25%布比卡因0.15ml行右侧星状神经节阻滞;N组解剖暴露右侧颈交感神经干后注入等容量的生理盐水,给药结束后15min三组大鼠均实施剖腹探查术,维持手术时间25min。分别于星状神经节阻滞前(T1)、手术结束时(T2)、术后12h(T3)、24h(T4)和48h(T5)采血样检测血清S100β蛋白和神经元特异性烯醇化酶(NSE)浓度;术后12、24、72h每组随机取两只大鼠处死,取海马组织,光镜下观察海马组织病理学变化;余大鼠进行Morris水迷宫实验,测定认知功能。结果与T1时比较,T2~T5时C组和N组血清S100β、NSE浓度明显升高(P〈0.05);S组T2、T3时血清S100β浓度和T2~T4时NSE浓度明显升高(P〈0.05);S组L、L时血清S100β浓度明显降低(P〈0.05)。与C组比较,T3~T5时N组和S组血清S100β和T2~T5时S组NSE浓度明显降低(P〈0.05)。与N组比较,T2~T5时S组血清S100β和NSE浓度明显降低(P〈0.05)。与第1天比较,第2~5天三组大鼠逃避潜伏期明显缩短(P〈0.05)。与C组比较,第3~5天N组和S组逃避潜伏期缩短(P〈0.05)。与N组比较,第2~5天S组逃避潜伏期缩短(P〈0.05)。穿越平台S组(6.9±1.8)次,明显多于N组(3.5±1.3)次和C组(2.8±0.8)次(P〈0.05)。结论星状神经节阻滞可减缓老龄大鼠术后血清S100β及NSE浓度上升水平,改善术后脑组织病理学变化,从而改善老龄大鼠术后认知功能。Objective To investigate the effect of stellate ganglion block (SGB) on serum S100β,NSE and postoperative cognitive dysfunction in aged rats. Methods Forty-eight aged male SD rats, 18-20 months, weighing 500-700 g were, randomly divided into 3 groups (n = 16 each) : in group C, right cervical ganglion were exposed and splited but not block; in group S, right cervical ganglion were exposed and spiked, then blocked with 0. 25% bupivacaine 0.15 ml; in group N, right cervical ganglion were exposed and spiked, then injected the same amount of saline. All rats underwent exploratory laparotomy and the duration of operation was 15 miru Blood collection to test serum S100β and NSE at the time before SGB (T1), at the end of operation(T2), 12 h (T3), 24 h (T4) and 48 h after operationb(T5). Two rats were killed at 12 h, 24 h and 48 h after operation in each group, their hippocampus were observed for the pathological changes. Cognitive function were assessed by morris water maze test on others rats. Results Compared with T1, serum concentrations of S100β and NSE were significantly increased in groups C and N from T2-T5 (P〈0.05) ; serum concentrations of S100β at T2 and T3 time points and concentration of NSE from T2-T4 were significantly elevated; whereas serum concentration of S100β at T4 and T5 were significantly decreased in group S (P〈0.05). Compared with group C, serum concentrations of S100β were significantly decreased from T3-T5 in groups N and S; and NSE concentrations in group S from group T2-T5 were also significantly decreased (P〈0. 05). Serum concentrations of S100β and NSE were significantly reduced from T2-T5 as compared with group N (P〈0. 05). Compared with day 1, the escape latency was significantly shortened on 2-5 days after operation in three groups (P〈0.05).
