新型口腔护理刷预防心内直视手术后患者相关并发症的效果

摘　　要：目的寻求一种更有效、便捷的口腔护理工具,提高口腔护理质量,减少心内直视手术术后患者口腔和肺部并发症。方法按入院先后将2011年6-12月在广州军区广州总医院行心内直视手术的130例患者分为对照组和观察组各65例,对照组应用传统口腔护理包,观察组采用新型口腔护理刷,比较两组患者口腔护理前后口腔黏膜损伤、口腔异味、口腔溃疡、清洁度、并发症发生情况及护士完成操作的时间等。结果护理前,两组患者的口腔异味、口腔溃疡、黏膜损伤及清洁度情况经比较,差异均无统计学意义(均P>0.05),与对照组患者相比,观察组患者的口腔异味、口腔清洁度显著优于对照组患者(均P<0.01);VAP的发生率显著低于对照组(P<0.05);护士操作准备时间及操作时间均短于对照组(均P<0.01)。结论新型口腔护理刷在心内直视术后患者口腔护理中应用对减少口腔和肺部并发症有积极作用。Objective To explore the effects of a new type of oral care brush on prevention of ventilator-associated pneumonia （VAP） in patients undergoing open heart surgery,so as to improve the nursing quality of oral care and decrease incidence of pulmonary complications. Methods Totally 130 patients underwent open heart surgery from June 2011 to December 2011 were divided into control group and observational group. The traditional oral cavity nursing bags were used in control group and a new type of oral care brush was used in observational group. Oral cavity mucous membrane damage, bad breath, oral ulcer, cleanliness, complications, comfort level and operating time of nursing were compared between two groups. Results No significant difference was found in bad breath, oralulcer, Oral cavity mucous membrane damage and cleanliness between the two groups before nursing interven- tion （P〈0.05）. Compared with the control group, bad breath, cleanliness in the observational group were signifi- cantly better （P〈0.01）, incidence of VAP in the observational group was significantly lower （P〈0.05）, prepare time and operating time in the observational group were significantly lower（P〈0.01）. Conclusion Application of new type of oral nursing brush will be beneficial to the VAP in oatients underwent open heart surgery.
