原发干燥综合征伴肺间质疾病患者临床特点分析

摘　　要：【目的】探讨原发干燥综合征（pSS）伴肺间质疾病（ILD）患者的临床表现、肺部高分辨率CT（HRCT）及实验室指标的特点。[A-法】收集pSS患者193例，依据HRCT结果判断是否合并ILD，分为单纯pSS组和pSS-II。D组，比较两组的临床特点。【结果]pSS—ILD组患者年龄、发病年龄及病程均显著高于单纯pSS组（P〈0．01）；pSII。D组患者咳嗽发生率、抗心磷脂抗体（ACLA）的阳性率明显高于单纯pSS组（P〈0．05）；pSS-ILD组患者的7G百分率、IgG、纤维蛋白原（Fg）、C反应蛋白（CRP）、血沉（ESR）的水平均高于单纯pSS组（P〈0．05），而血清白蛋白（Alb）水平低于单纯pSS组（P〈0．05）。【结论】年龄较大的pSS患者易伴发ILD，pSS患者中若伴有咳嗽症状需警惕是否伴有ILD；ACLA，7-G百分率及IgG、补体C3、Alla、CRP、ESR、Fg水平可作为预测pSS伴发ILD的临床指标。[Objective]To explore the clinical manifestation and the features of pulmonary HRCT and labo- ratory indices of primary Sjogren syndrome（pSS） with interstitial lung disease（ILD）. [Methods] Totally 193 patients with pSS were collected. Based on the results of HRCT, pSS with or without ILD was judged. All patients were divided into simple pSS group and pSS-ILD group. Clinical features were compared between two groups. [Results] The age, onset age and disease duration in pSS ILD group were higher than those in simple pSS group（ P d0.01）. The incidence of cough and the positive rate of anticardiolipin antibody（ACLA） in pSS-ILD group were higher than those in simple pSS group（ P d0.05）. The levels of IgG, y-G, fibrinogen （Fg）, C-reactive protein（CRP） and erythroeyte sedimentation rate（ESR） in pSS-ILD group were higher than those in simple pSS group（ P d0.05）, while the levels of serum alhumin（Alb） in pSS-ILD group were lower than those in simple pSS group（ P 〈0.05）. [Conclusion] The elder patients with pSS are prone to accompany with ILD. The patients with pSS who have cough should be alerted to have ILD. ACLA, the percentage of r- G and the levels of IgG, C3, Alb CRP, ESR and Fg can be used as the clinical indices to predict the occurrence of ILD in pSS patients.
