鼻内镜下鼻窦炎手术中实施中鼻甲外侧壁切除的疗效分析

摘　　要：【目的】探讨鼻内镜下鼻窦炎手术中实施中鼻甲外侧壁切除的疗效。【方法】将80例行鼻内镜下鼻窦炎手术的患者随机分为对照组与观察组，对照组行单纯的鼻窦炎开放手术，观察组在对照组治疗的基础上行中鼻甲外侧壁切除术。比较两组术后换药平均次数、临床疗效、治疗后sNOT-20评分及不良反应等。【结果】对照组术后平均换药次数为（6．19±1．04）次，显著高于观察组（1．34±0．22）次，总有效率72．50％显著低于观察组92．50％，其差异均有显著差异（P〈O．01）；两组治疗前sNOT-20评分比较无统计学差异（P〉0．05），但观察组治疗后SNOT-20总分显著小于对照组治疗后（P〈0．05），两组不良反应发生率比较无统计学差异（P〉O．05）。【结论】鼻内镜下鼻窦炎手术中实施中鼻甲外侧壁切除的临床疗效显著，安全性较高，值得在临床推广应用。[Objective] To explore the efficacy of lateral wall resection of middle turbinate during endo- scopic sinusitis surgery. [Methods] Totally 80 patients undergoing endoscopic sinusitis surgery were randomly divided into control group and observation group. The control group underwent single open sinusitis surgery, while the observation group underwent lateral wall resection of middle turbinate on the basis of the treatment of control group. The average frequency of postoperative dressing, clinical efficacy, SNOT-20 score and ad- verse reaction after treatment were compared between two groups. [Results] The average frequencies of post- operative dressing in control group were （6.19 ± 1.04） times, which were significantly higher than those in observation group（1. 34 ± 0. 22） times]. Total effective rate of control group was 72. 50%, which was markedly lower than that of observation group（92.50 %）, and there were significantly differences between two groups（ P d0. 01）. There was no significant difference in SNOT-20 score between the two groups before treatment（ P 〉0.05）, but SNOT-20 score of the observation group after treatment was significantly lower than that of the control group（ P 〈0.05）. There was no significant difference in the incidence of adverse reac- tions between the two groups （ P 〉0.05）. [Conclusion]Endoscopic sinus surgery in the lateral wall resection of middle turbinate during endoscopic sinusitis surgery has significant clinical efficacy and high safety, so it is worthy of clinical promotion.
