摘 要:目的 探讨2型糖尿病患者不同水平的糖化血红蛋白(HbA1c)对心脏结构和功能的影响.方法 选择2型糖尿病住院患者389例,依据HbA1c水平分为3组,即HbA1c〈7%组59例,7%~9%组115例,HbA1c≥9%组215例.根据超声心动图结果评价心脏的结构和功能.结果 与HbA1c〈7%组比较,HbA1c≥7%的2组患者舒张末期左室后壁厚度(LVPW)、相对室壁厚度(RWT)、室间隔厚度(IVS)、左室质量指数(LVWI)明显升高,左室舒张早期二尖瓣最大流速(E)、舒张晚期二尖瓣最大流速(A)及E/A比值明显降低,差异均有统计学意义(P﹤0.05,P﹤0.01),而HbA1c〉9%组E值、A值、E/A值与7%~9%组比较差异亦有统计学意义(P〈0.05,P〈0.01).结论 2型糖尿病患者HbA1c≥7%时,其心脏结构和功能会出现不同程度的异常,其异常程度随着HbA1c的增加而加重,提示合并心血管疾病的可能性增加.Objective To study the effects of the different glycated hemoglobin(HbA_(1c)) on the cardiac structure and function in type 2 diabetes mellitus(T2DM).Methods 389 patients with T2DM were categorized as:HbA_(1c) 7%group(n = 59 cases),HbA_(1c) 7%~9%group(n = 115 cases) and HbA_(1c) 9%group(n =215 cases).The cardiac structure and function were evaluated according to the results of echocardiographic measurements.Results Left ventricular posterior wall thickness(LVPW),relative wall thickness(RWT),interventricular septal thickness(IVS),left ventricular mass index(LVMI) of type 2 diabetes mellitus with HbA_(1c) ≥7%were higher than HbA_(1c) 7%group(P 0.05 or P 0.01),meanwhile the ratio of E/A was much lower,especially in HbA_(1c) 9%group(P 0.05 or P 0.01).Conclusion In type 2diabetic patients with HbA_(1c) ≥7%,the cardiac structure and function were involved.With the increase of HbA_(1c),the impairments were more prominent,which may predict cardiovascular disease.
