摘  要:目的比较七氟醚单独与复合笑气吸入用于幼儿全凭吸入麻醉诱导的效果。方法将40例择期拟在全身麻醉下行腭裂修补术的患儿随机分为S组和SN组各20例,ASA分级Ⅰ级。S组全凭7%七氟醚吸入麻醉诱导,SN组复合7%七氟醚和50%笑气吸入麻醉诱导。观察并记录患儿诱导前(T0)、气管插管即刻(T1)、气管插管后2 min时(T2)的平均动脉压(MAP)、心率(HR)、脉搏血氧饱和度(SpO2)、脑电双频指数(BIS)值和吸入麻醉药的肺泡最小有效浓度(MAC)值;记录患儿睫毛反射消失时间、气管插管时间、气管插管时Cooper评分。结果两组各时点MAP、HR、SpO2比较,P均>0.05;S组患儿T1时BIS值与MAC值分别为49.75±7.46、2.51%±0.32%,SN组分别为41.33±2.12、3.04%±0.48%,P均<0.05;S组患儿睫毛反射消失时间和气管插管时间分别(54.25±8.95)、(262.50±54.97)s,SN组分别为(32.00±4.24)、(165.33±44.71)s,P均<0.05。两组气管插管时Cooper评分比较,P>0.05。结论与七氟醚单独吸入麻醉诱导相比,七氟醚复合笑气用于幼儿全凭吸入麻醉诱导,能缩短麻醉诱导时间,且心血管系统平稳,插管条件良好。Objective To compare the effects of sevoflurane alone and sevoflurane/nitrous oxide inhalation on children's general anesthesia induction.Methods A total of 40 ASA I children undergoing cleft palate surgery under general anesthesia were randomly divided into sevoflurane group (group S) and sevoflurane with nitrous oxide group (group SN).The children in group S were inducted by 7% sevoflurane alone,while children in the group SN were inducted by 7% sevoflurane with 50% nitrous oxide.Mean arterial pressure (MAP),heart rate (HR),pulse oxygen saturation (SpO2),bispectral index (BIS) and minimal alveolar concentration (MAC) value were recorded at the following time points:before anesthesia induction (T0),at the time of endotracheal intubation (T1) and 2 minutes after endotracheal intubation (T2).The time of loss of eyelash reflex,endotracheal intubation and the endotracheal intubation conditions were recorded as well.Results No significant difference was found in MAP,HR and SpO2 at different time points between two groups (all P >0.05).The BIS value of group S and group SN was 49.75 ± 7.46 and 41.33 ± 2.12,while the MAC value was 2.51% ± 0.32% and 3.04% ± 0.48% at T1 (all P < 0.05).The time of loss of eyelash reflex and endotracheal intubation was (54.25 ± 8.95) s and (262.50 ± 54.97) s in group S,(32.00 ± 4.24) s and (165.33 ± 44.71) s in group SN (all P <0.05).The endotracheal intubation conditions between two groups were not significantly different (P > 0.05).Conclusion Compared with the sevoflurane alone inhalation,sevoflurane with nitrous oxide inhalation on children's general anesthesia induction can shorten the time for induction and with stable cardiovascular system and good endotracheal intubation conditions.
