肾微小动脉血流参数在糖尿病肾病诊断中的应用价值

摘　　要：目的探讨彩色多普勒检测糖尿病肾病肾内末梢小动脉血流动力学改变,在糖尿病肾病(DN)临床诊断中的应用价值。方法应用彩色多普勒超声对40例健康成人(健康对照组)、20例1型糖尿病患者、40例2型糖尿病患者(分为早期DN组和临床DN组)肾微小动脉血流动力学参数进行比较分析。结果临床DN组与早期DN组、正常对照组比较,肾微小动脉血流参数值搏动指数(PI)和阻力指数(RI)明显增高,收缩期峰值流速(PSV)、舒张末期流速(EDV)明显降低(P<0.05);早期DN组与正常对照组比较,PI、RI增高,PSV、EDV降低(P<0.05)。结论彩色多普勒检测肾内微小动脉各血流参数在诊断糖尿病肾病肾脏损害程度中有重要临床参考价值。Objective To explore the value of Doppler indices of various small peripheral renal artery＇s hemodynam- ics in patients with diabetes nephropathy and the diagnostic value of the color Doppler ultrasonography. Methods Hemo- dynamics of 40 healthy subjects （normal group） and 20 patients with type 1 diabetes ,40 patients with type 2 diabetes（ clin- ical DN group and early DN group） were analyzed. Results Comparing the hemodynamics of the clinical DN group,the early DN group and the normal group ,pulsatility index（PI） and resistive index （RI） in clinical DN group were higher than the normal group and the early DN group, and the peak systolic velocity（PSV） and end diastolic velocity（EDV） were lower （ P 〈 0.05 ）. Comparing the early DN group and the normal group, the hemodynamies PI and RI in the early DN group were higher than the normal group, and PSV and EDV were lower （ P 〈 0.05 ）. Conclusion The hemodynamics of the small peripheral renal artery in patients with diabetes nephropathy and renal failure by Color Doppler ultrasound may be an impor- tant diagnostic method for clinical diagnosis.
