乳腺密度的影响因素及其在乳腺疾病中的诊断价值

摘　　要：分析年龄、是否绝经对乳腺密度的影响及乳腺密度与诊断敏感性的关系。方法 1432例患者随机先后接受X线摄影和全乳腺超声检查。按照BI-RADS分类对乳腺密度进行分类,利用χ2 检验进行统计分析。结果 年龄与乳腺密度呈负相关（r 值为-0.543,P=0.000）。绝经女性乳腺密度显著低于非绝经女性。≤30岁,31~40岁,41~50岁,51~60岁及≥61岁组致密性腺体的比例分别为91.3%、87.9%、78.2%、41.2%、12%。腺体类型1~4型X线诊断敏感性分别为100.0%,95.2%,88.1%及80.5%,有显著性差异（χ2=36.503,P=0.000,r 值为-0.220）;超声敏感性分别为100%,93.4%,95.9%及97.7%（χ2=9.202,P=0.020,R 值为0.027）,有差异,但影响不大。对于致密性腺体,超声敏感性显著高于X 线（96.7%与84.7%,χ2 =33.367,P=0.000）。结论 乳腺密度随年龄增长而减低。乳腺密度影响X线诊断敏感性,但不影响超声诊断。Objective To analyze factors affecting breast density and to study the relationship between breast density and diagnostic sensitivity. Methods 1 432 female were rolled in this study who had digital mammography （DM） and ultrasonography （US）. All images were assessed according to BI-RADS categories. X2 test was used for statistics. Results There was a significant inverse relationship between age and breast density （r=-0. 543, P = 0. 000）. The breast density in postmenopausal women was significant lower than that in premenopausal women （X2 =382. 856,P=0. 000）. For women 430 years, 31-40 years, 41-50 years, 51-60 years, and ≥61 years, the proportion with dense breast tissue was 91.3%, 87.9%, 78.2%, 41.2% and 12%, respectively. Sensitivity of DM in women with breast density type 1, 2, 3, and 4 was 100.0%, 95.2%,88.1% and 80.5%, respectively（x2 = 36. 503, P=0.000,r=-0.220）; and those of US, 100%,93.4%,95.9% and 97.7%, respectively （ X2=9.202,P=0.020,r=0. 027）. There was significant difference between DM and US for dense breast （ 96.7% versus 84.7%, X2 = 33. 367, P= 0. 000 ）. Conclusion The breast density decrease with patient age. Breast density has effect on DM sensitivity, but not on US.
