双源CT双能量融合图像在直肠癌应用中的研究初探

摘　　要：探讨双源CT双能量线性融合（linearblending,LB）和非线性融合（non-linearblending,NLB）图像在直肠癌分期中的临床应用价值。方法 回顾性分析43例经手术病理证实为直肠癌患者的双能量图像,将140kV和100kV2种能量图像进行融合,生成0.3LB、0.5LB、NLB1（λ=150HU,ω=200HU）、NLB2（λ=40HU,ω=200HU）和NLB3（λ=60HU,ω=200HU）5组图像,比较各组图像的直肠癌病灶对比噪声比（contrastnoiseratio,CNR病灶）、髂外动脉信噪比（signaltonoiseratio,SNR髂外）及图像背景噪声。结果 NLB2、NLB3图像的CNR病灶、SNR髂外均高于0.3LB组图像（P 均〈0.05）,而0.5LB与NLB1、NLB2和NLB3图像两两间CNR病灶、SNR髂外的差异均无统计学意义（P 均〉0.05）。3组NLB图像的背景噪声均低于LB组图像,差异均具有统计学意义（P 均〈0.05）。结论 NLB2、NBL3组图像可提高直肠癌病灶的显示效果,在直肠癌分期中具有潜在的临床应用价值。Objective To investigate the clinical value of dual energy linear blending（LB） and non-linear blending（NLB） images in rectal cancer staging using dual-source computed tomography. Methods The dual energy CT images of 43 patients with primary rectal cancer proved by surgery and pathology were retrospectively analyzed. Images of 140 kV and 100 kV were fused to derived ima ges in five groups, including 0.3LB, 0.5LB, NLBI（λ=ISO HU,ω=200 HU）, NLB2（λ=40 HU,ω=200 HU） and NLB3（λ=60 HU,ω=200 HU） images. The contrast to noise ratio （CNR） of rectal cancer, signal to noise ratio （SNR） of external iliae artery and the background noise of images were compared. Results The CNR of primary lesions, SNR of external iliac artery on NLB2 and NLB3 images were significantly higher than those on 0.3LB images（ P〈0.05}, but there were no statistical difference between group 0.5LB and NLB1, NLB2, NLB3 images in either CNR of primary lesions or SNR of external iliae artery（ P〉0.05 ）. The background noise on three groups of NLB images were significantly lower than those on LB images （P（0.05）. Conclusion NLB2 and NLB3 im ages can improve depiction of primary Iesions of rectal cancer, which could lead to the potential value in preoperative staging.
