住院糖尿病患者与非糖尿病患者痰液标本中肺炎链球菌的分离情况

摘　　要：目的了解住院糖尿病患者与非糖尿病患者肺炎链球菌感染情况,为疾病防控提供科学依据。方法从广州市某综合医院实验室管理系统数据库,调取2011-2012年所有住院病例痰液标本病原菌培养及药敏试验结果。从该医院病案管理系统数据库,调取2011-2012年所有住院病人的基本资料,出院诊断包含"糖尿病(E10-E14)"的为糖尿病患者。将上述2个数据库的数据按住院号、姓名、性别、出生日期/年龄进行匹配、整合,分析糖尿病患者与非糖尿病患者痰液标本肺炎链球菌分离情况及耐药性。结果共分离细菌1440株,真菌480株。仅在7名非糖尿病患者中分离出7株肺炎链球菌,其中6株对红霉素耐药,2株对青霉素耐药,全部对左旋氧氟沙星或莫西沙星敏感。结论肺炎链球菌在住院糖尿病患者与非糖尿病患者痰液标本中的培养阳性率低。Objective To study S. pneumoniae infection in hospitalized diabetic and non-diabetic patients, and to provide scientific evidence for disease control and prevention. Methods From the laboratory database of a general hospital in Guangzhou, results of bacterial and fungal cultures of the sputum specimens collected from the hospitalized patients in 2011 to 2012 and related antibiotic susceptibility testing results were retrieved. From the patient record database of the same hospital, patients discharged in 2011 to 2012 with or without a diagnosis of ＂diabetes mellitus （E10-E14）＂ were identified. These two data sets were then merged based upon common variables such as admission number, name, gender and date of birth/age. Isolation of S. pneumoniae and antibiotic susceptibility testing results were compared between diabetic and nondiabetic patients. Results ＇A total of 1 440 bacterial strains and 480 fungal strains were isolated. Only 7 strains of S. pneumoniae were isolated from the sputum specimens of 7 non-diabetic patients. 6 strains were resistant to Erythromycin,2 strains were resistant to Penicillin,and all were sensitive to Levofloxaein or Moxifloxacin. Conclusion The culture positivity rate for S. pneurnortiae in the sputum specimens from either diabetic or non-diabetic hospitalized patients was low.
