中小学接种甲型H1N1流行性感冒疫苗的安全性和保护效果观察

摘　　要：目的观察海珠区中小学接种甲型H1N1流行性感冒疫苗(简称甲流疫苗)的安全性和流行病学保护效果。方法采用描述流行病学、队列研究方法,对2116名接受甲流疫苗接种(接种组)和2762名未接受甲流疫苗接种(对照组)的在校中小学随访1年,观察甲型H1N1流感疫苗的安全性和流行病学保护效果。结果接种甲型H1N1流感疫苗师生中,累计报告不良反应人数为4例,报告率为189/10万,所有接种对象在接种后半年内未报告格林巴例综合征(GBS);接种组流感样病例发病率为3.2%,对照组发病率为4.5%,疫苗保护率为28.9%,保护效果指数为1.39。接种组甲流病例发病率为0.24%,对照组发病率为0.72%,疫苗保护率为67.37%,保护效果指数为3.06。结论甲型H1N1流感疫苗具有良好的安全性和较好的流行病学保护效果。Objective To evaluate the safety and protective effect of influenza A （H1N1） vaccination among primary and middle school students and teacher in Haizhu district,Guangzhou.Method A one-year follow up cohort study was used.2 116 students and teacher who had been injected influenza A （H1N1） vaccine were divided in the vaccination group and 2 762 students and teacher who had not received vaccination divided in the non-vaccination （control） group.We followed these students for one year to evaluate the safety and protective effect.Results 4 adverse reaction cases （reporting rate was 189/100000） were reported among the vaccination group during the whole study.No vaccination student had been reported Guillain-Barre Syndrome within six months.The rate of influenza-like symptom cases waslower in the vaccination group （3.2%） than that of the non-vaccination group （4.5%）.The protection rate was 28.9% and the protection index was 1.39.The incidence of influenza A （H1N1） was 0.24% in the vaccination group,while it was 0.72% in the nonvaccination （control） group.The protection rate of influenza A （H1N1） was 67.37% and the protection index was 3.06.Conclusion The influenza A （H1N1） vaccine was safe and had good protective effect.
