半胱氨酸蛋白酶抑制剂C与冠心病及高血压患者心室重塑关系的研究

摘　　要：目的分析高血压、冠心病患者半胱氨酸蛋白酶抑制剂C(Cystatin C)与心室重塑程度评价指标的相关性,探讨Cystatin C在心室重塑监测中的意义。方法原发性高血压、冠心病患者120例,按临床病例诊断将其分为原发性高血压组43例、冠心病组19例、冠心病合并高血压组39例,另选41名健康志愿者作为对照组。采用乳胶颗粒增强比浊法检测研究对象血清Cystatin C水平,并分析其与心脏彩超参数、左心室心肌质量指数(LVMI)以及N端脑钠肽前体(NT-proBN)在反映心室重塑程度时的监测意义。结果原发性高血压组中Cystatin C与LVMI、NT-proBNP呈正相关,冠心病组中Cystatin C与NT-proBNP呈正相关;高血压组及冠心病合并高血压组LVMI明显高于冠心病组及对照组;冠心病、高血压、冠心病合并高血压疾病三组之间的Cystatin C水平差异无统计学意义。结论高血压及冠心病患者的Cystatin C水平与NT-proBN呈正相关,Cystatin C在高血压心室重塑监测中有一定的参考价值,对于临床综合评价患者的心室重塑程度有重要意义。Objective The present study was aim to analyze the clinical correlation between Cystatin C and some indicators of cardiac remodeling in essential hypertension （EH） and coronary heart diseases （CHD）, and to detect the clinical significance of Cystatin C used to monitor the process of cardiac remodeling for EH and CHD patients. Methods This study included 120 EH and/or CHD patients, and 19 patients without cardiovascular disease as control. Cystatin C was detected by the latex enhanced turbidimetric method, and the correlation among Cystatin C, NT-proBNP, and indicators from Doppler echocardiography including Left ventricular mass index （LVMI） was analyzed. Results A positive linear relationship between Cystatin C and LVMI or NT-proBNP was demonstrated in EH group. In CHD group, Cystatin C was also related with NT-proBNP. LVMI were higher in patients of EH group and CHD patients complicated by EH than that of patients in CHD group and control group. Cystatin C was not significantly different among patients of EI-I group, CHD group and CHD patients complicated by EH. Conclusion Cystatin C might be a valuable biomarker in monitoring the process of hypertensive cardiac remodeling, which could provide an extra method to assess the cardiac remodeling process with other traditional indicators.
