活血杀胚消癥法对早孕大鼠妊娠滋养细胞影响的实验研究  

An Experimental Study on the Effect of Blood-invigorating and Embryo-killing and Disease-eliminating Prescription on Gestational Trophocyte of Pregnant Rats

摘　　要：目的通过观察活血杀胚消癥法对妊娠大鼠滋养细胞形态和功能的影响,间接探索中药治疗异位妊娠的机理。方法将造模成功后的早孕大鼠随机分为4组:中药组、西药组、中西药结合组、空白组,分别给以活血杀胚消癥方煎剂灌胃、甲氨蝶呤肌注、及中西药联合应用,空白组以等量生理盐水灌胃。给药第8天查血β-HCG并观察滋养细胞组织病理学改变及细胞超微结构变化。结果中药组、西药组、中西药结合组、空白组血人绒毛膜促性腺激素(β-HCG)均值分别为:(8.55±4.103)、(9.08±3.273)、(3.86±1.715)、(16.6±3.634)mlU/mL,中药组、西药组分别与空白组比较有差异(P<0.05),中药组和西药组比较无差异(P>0.05),中西药结合组分别与另外3组比较有显著性差异(P<0.01)。3个用药组与空白组比较均出现滋养细胞细胞组织病理学及细胞超微结构的改变:表现为细胞间隙增大、排列紊乱,炎性细胞浸润,胚囊正常形态消失;细胞核内出现大量染色质,线粒体肿胀,部分线粒体嵴紊乱甚至消失。其中以中西药结合组改变最显著。结论活血杀胚消癥法作用机理可能是通过改变滋养细胞的超微结构,影响滋养细胞的数量、形态、分泌功能,促使炎症细胞浸润,致胚胎组织发育停滞而起到杀胚作用。Objective:To observe the effect of blood-invigorating and embryo-killing and disease-eliminating prescription on the trophocyte morphology and function of pregnant rats and to indirectly explore the ectopic pregnancy mechanism by TCM.Methods:Early modeling pregnancy rats were randomly divided into four groups,a TCM group,a western medicine group,a Chinese-western medicine group and a control group and were administrated with the decoction of blood-invigorating and embryo-killing and disease-eliminating prescription,methotrexate intramuscular and Chinese-western integration medicine while non-drug group was administrated with equivalent saline.In the 8th day,β-HCG blood was tested and the pathological changes and the cell ultrastructure changes of trophoblastic tissue were observed.Results:β- HCG of the TCM group,the western medicine group,Chinese-western medicine group and non-drug group respectively mean(8.55±4.103),(9.08±3.273),(3.86±1.715)and(16.6±3.634)mlU/mL.Compared with the non-drug group, there was difference(P<0.05)between the TCM group and the western medicine group,but the comparison between the TCM group and the western medicine showed no difference(P>0.05).Compared with the three treatment groups,the Chinese-western medicine group had significant difference(P<0.01).Compared with the non-drug group,the changes of the pathological changes and the cell ultrastructure changes of trophoblastic tissue appeared in the three groups.The changes were that the cell gap increased,arrangement disorgnized, inflammatory cell infiltrated and sac normal morphology disappeared.Massive chromatin appeared within nucleus cells,with mitochondria swelling,and some mitochondria were in disorders or even disappeared.The change of the Chinese-western medicine group was the most significant.Conclusion:This blood-invigorating and embryo-killing and disease-eliminating prescription may kill embryo and cause the stagnation of embryonic tissue development by altering the ultrastructure of trophoblast cells,affecting the numb
