摘 要:目的通过NT-3转染的BMSCs联合去细胞异体神经修复大鼠坐骨神经缺损,观察神经移植体传导速度及靶器官功能恢复。方法 60只实验大鼠随机分为4组,各组均造成1 cm长左下肢坐骨神经缺损。A组应用自体神经修复,B组应用去细胞异体神经,C组应用去细胞异体神经+BMSCs,D组应用去细胞异体神经+转染NT-3的BMSCs。移植后1、4、8周肉眼观察大鼠足部溃疡形成及愈合情况,移植后4、8、12周切片观察移植体再生情况,肌电图检测神经传导速度及小腿三头肌恢复率。结果移植1周后,各组均有下肢溃疡形成,4周后恢复;8周后切片观察,A组及D组神经纤维直径及髓鞘数量均明显优于B、C组;12周时,神经传导速度A组明显优于其余3组,D组优于其余2组(P<0.05);小腿三头肌湿重恢复率D组也明显优于其余2组(P<0.05)。结论去细胞同种异体神经联合转染NT-3的BMSCs作为神经缺损的修复材料,不仅能够提供良好的支架作用,有效地提高神经传导速度的恢复,对靶器官的恢复也具有较明显的促进作用。Objective The BMSCs transfected by NT-3 combined with chemical extracted nerve graft of homogeneity repairing sciatic nerve defets of rats to observe the conduction velocity of nerve allograft and functional recovery of target organ. Method The 60 rats randomly divided into four groups. Each group all was subjected to 10mm sciatic nerve gap on left leg, then receired following methods to be repaired, respectively. Group A: useing the autologous nerve; Group B: the chemical extracted acellular nerve allograft; Group C: the chemical extracted aeellular nerve allograft combined with BMSCs; Group D: the BMSCs transfected by NT-3 combined with chemical extracted nerve graft of homogeneity. At 1 week, 4 weeks and 8 weeks after transplantion the ulcer formation and healing of rat paw, at 4 weeks, 8 weeks and 12 weeks after transplantion the regeneration of nerve transplantion was observed respectively by naked eyes or tissue biopsies; the nerve conduction velocity was detected by electromyography, and the recovery rates of triceps surae was calculated. Results One week after transplantion: each group had ulcer formation of rat paw, and healing at 4 weeks; the tissue biopsies at 8 weeks showed that Group A and D was much better than Group B and C on nerve fiber diameter and the number of myelin sheath; at 12 weeks after transplantion, the nerve conduction velocity of Group A was much better than other groups, and Group D was better than B and C (P〈0.05); the triceps surae recovery rates of Group D also was better than B and C (P〈0.05). Conclusion The BMSCs transfected by NT-3 combined with chemical extraeted nerve graft of homogeneity as materials used for repairing the never gap not only offer the functions of scaffold, increase the recovery rates of nerve conduction velocity effectively, but also effected on the recuperated of target organ.
