参附汤对慢性心力衰竭患者血浆B型钠尿肽的影响

摘　　要：目的观察参附汤对慢性心力衰竭患者血浆B型钠尿肽(BNP)浓度的影响。方法将116例慢性心力衰竭患者随机分为治疗组60例和对照组56例,两组均采用最佳西医治疗方案,并在此基础上分别加用参附汤和安慰剂汤,12周为1个疗程,共治疗2个疗程。分别于治疗前、治疗第1个疗程、治疗第2个疗程检测血浆BNP浓度,并进行统计学分析。结果治疗前,两组患者血浆BNP水平比较差异无统计学意义(P>0.05);治疗2个疗程后,两组患者血浆BNP水平分别与治疗前比较,差异均有统计学意义(P<0.01),两组患者随着治疗疗程的增加,血浆BNP水平逐渐降低(P<0.01);治疗组血浆BNP水平降低幅度高于对照组(P<0.01)。中医辨证分析,两组心肾阳虚证、气虚血瘀证患者血浆BNP水平分别与治疗前比较,差异均有统计学意义(P<0.01),两组心肾阳虚证、气虚血瘀证患者随着治疗疗程的增加,血浆BNP水平逐渐降低(P<0.01);治疗组心肾阳虚证、气虚血瘀证患者血浆BNP水平降低幅度高于对照组(P<0.01)。治疗组心气虚证与对照组比较差异无统计学意义(P>0.05),治疗组和对照组心气虚证与本组各时段心肾阳虚证、气虚血瘀证比较,差异均有统计学意义(P<0.01);治疗组和对照组心肾阳虚证各时段比较,差异有统计学意义(P<0.01)。结论参附汤能显著降低心肾阳虚证、气虚血瘀证慢性心力衰竭患者血浆BNP浓度,保护心脏,改善心功能。Objective To observe the effect of Shenfu Decoction on plasma B type natriuretic peptide（BNP） of patients with chronic heart failure.Methods 11{5 patients with chronic heart failure were randomly divided into the treatment group（ n =60） and the control group（ n = 50）. Both groups were treated by the best Western medicine treatment program. Based on the treatment the treatment group were added Shenfu Decoction and the control group were added Placebo Decoction.All patients were treated for 2 courses with 12 weeks in a course.Then, the plasma BNP concentration of patients was tested before treatment, in the first course of treatment and the second course of treatment respectively.Statistical analysis was also carried out.Results Before treatment, the plasma BNP levels of these two groups showed no significant difference（ P 〉0.05）. After 2 courses of treatment,the plasma BNP levels of both groups was significantly different from those before treatment （ P 〈0.01）.The BNP levels of both groups decreased gradually with the increase of treatment courses（ P〈0.01）.However, the plasma BNP level of the treatment group decreased more significantly than the control group（ P 〈0.01）.TCM dialectical analysis showed that patients with syndrome of yang deficiency of heart kidney and syndrome of qi deficiency of blood stasis in both groups had gradually decreasing BNP levels （ P 〈0.01） with the increase of treatment courses, which showed significant difference from that before treatment（ P〈0.01）.But those in the treatment group had more obvious decrease than the control group （ P 〈0.01）.No significant difference was found in the comparison of the syndrome of qi deficentcy of heart between the patients in the two groups（ P 〉0.05）.And the difference of syndrome of yang deficiency of heart kidney and syndrome of qi deficiency of blood stasis during different periods within groups had statistical significance（ P〈0.01）.The comparison of syndrome of yang deficiency of hear
