数字血管减影引导锁骨下静脉置管和PICC应用对比观察

摘　　要：目的探讨数字血管减影(digital subtraction angiography,DSA)引导锁骨下静脉置管技术与经外周静脉行中心静脉置管(Peripheral inserted central catheter,PICC)的应用对比观察。方法 180例置管患者随机分为两组,锁骨下静脉组90例在DSA引导下进行锁骨下静脉置管,PICC组90例行PICC术。术后观察两种方法置管成功率、并发症发生率及留置导管时间的长短。结果锁骨下静脉组置管成功率为95.6%,PICC组为96.6%,两组比较差异无统计学意义(P>0.05);PICC组术后并发导管相关性感染、静脉血栓并发症的例数较锁骨下静脉组多,组间比较差异有统计学意义(P<0.05)。术后两组导管留置时间比较差异无统计学意义(P>0.05)。结论 DSA引导锁骨下静脉置管穿刺技术定位准确、并发症少,值得临床推广应用。Objective To investigate and compare application of Digital Subtraction Angiography （DSA） in guidiag subclavian vein puncture and Peripheral Inserted Centual Catheter （PICC）.Methods 180 patients were randomly divided into two groups the DSA group （ n =90）and the PICC group （ n =90）. The former group underwent subclavian vein puncture ureter the guidance of DSA and the latter group underwent PICC.Tben, the catheter success rate, complications and catheter indwelling time of both groups were observed re,＇;pectively. Resuits The catheter success rate of the DSA group was 95.6% and that of the PICC group was 96.6% ,which showed no significant statisticat difference（ P 〉0.05）.There were more cases with infection related to catheter and complications of venous thrombosis in the PICC group.So it is significantly different between two groups in this aspect（ P 〈20.05）. Tbe post operative catheter indwelling time of two groups was not statistically different（ P 〉0.05）.Conclusion DSA guided subclavian vein puncture is accurate in positioning catheter and causes fewer compolications, so it is of great value for clinical application.
