摘 要:目的探讨核因子-B(NF-κB)在儿童紫癜性肾炎(HSPN)中的作用及黄芪对其的影响。方法选取35例HSPN患儿(HSPN组)和30例健康儿童(健康对照组)作为研究对象。抽取所有研究对象5 ml静脉血并提取外周血单个核细胞(PBMC),分别加入NF-κB刺激剂白介素-1(IL-1)、NF-B抑制剂对甲苯磺酰-L-苯丙氨酸氯甲基酮(TPCK)及黄芪注射液。经培养后离心,留细胞沉渣提取核蛋白,采用ELISA检测其NFB活性。结果 HSPN组PBMC NF-κB活性明显高于健康对照组(<0.01)。黄芪能显著降低HSPN组PBMC NF-KB活性(<0.01),但不影响健康对照组NF-κB活性(>0.05)。经IL-1B刺激后,HSPN组和健康对照组NF-B活性均显著增高(均<0.01),而黄芪能显著降低上述两组经IL-1作用后的NF-B活性(均<0.01)。经TPCK抑制后,HSPN组和健康对照组NF-κB活性均显著降低(均<0.01),而黄芪能显著提高上述两组经TPCK作用后的NF-B活性(均<0.01);黄芪对HSPN组PBMCNF-B活性的影响较健康对照组明显(<0.01)。结论 HSPN患儿PBMC的NF-κB活性上调,推测其参与HSPN的发病;黄芪能显著降低HSPN患儿PBMC NF-κB活性;黄芪对PBMC的NF-B信号途径具有双向调节作用。Objective To investigate the pathogenesis of peripheral blood mononuclear cells (PBMC)nuclear fac- tor-KB (NF-r,B)in children with Henoch -Schonlein purpura nephritis (HSPN)and the effect of astragalus on the ac- tivity ofNF-kB. Methods Thirty-five children with HSPN and thirty normal children were studied. Isolated PBMC were separated from 5 ml venous blood in asepsis condition. NF-r,B stimulator, NF-r,B inhabitor and astragalus were added into the different tubes of PBMC, respectively. The nuclear protein was extracted from the pellets and the op- tical density(A)values of nuclear protein were measured by enzyme-linked immunosorbent assay. Results The ac- tivity of PBMC NF-rB in HSPN group was higher than that in normal group (P 〈 0.01). In the normal culture con- ditions, astragahs could decrease the activity ofPBMC NF-rd3 in children with HSPN (P 〈 0.01), but did not affect that of normal children (P 〉 0.05). Astragalus could decrease the activity ofPBMC NF-r,B which had been stimulated by intedeukin-1 [3 (IL-]3) (P 〈 0.01)in HSPN group and normal group; and astragalus could make the activity of NF- rB elevated after which had been inhibited by N-tosyl-Phenylalanine hlormethyl-ketone (TPCK)(P 〈 0.01). A sig- nificant effect of astragalus on the activity ofPBMC NF-rd3 in children with HSPN was observed comparing with that of normal group (P 〈 0.01). Conclusions In children with HSPN, the activity ofPBMC NF-r,B is up-regulated. It is infering that NF-rA3 may participate in the pathogenesis of HSPN. Astragalus can decrease the activity of NF-rd3 in children with HSPN. Astragalus can regulate semaphoric approach of PBMC NF-rB bilaterally.
