下蒂真皮组织瓣结合乳房下皱襞切口在乳房肥大症治疗中的临床应用

摘　　要：目的 探讨乳房下蒂真皮组织瓣结合乳房下皱襞切口在乳房肥大症治疗中的临床应用.方法 选取2009年3月～～2012年3月收治的32例拟行巨乳缩小术的巨乳患者,采用乳房下皱襞切口切除大部分乳腺组织,保留中央腺体蒂及乳房下蒂真皮组织瓣以保证乳头乳晕血运,以乳房下蒂真皮组织瓣折叠组成乳房内部充填物,以乳房上极皮肤包被下蒂真皮组织瓣形成新的乳房外层皮肤.结果 术后随访6～～36个月,32例患者均对术后乳房形态满意,术后乳房切口不明显,乳头乳晕无坏死.讨论 乳房下蒂真皮组织瓣结合乳房下皱襞切口在乳房肥大的治疗中具有瘢痕隐蔽、乳房术后形态美观自然等优点,是一种治疗中重度乳房肥大的有效方法.Objective To investigate the clinical application of the inferior dermal Pedicle with inframammary fold incision technique in breast hypertrophy. Methods From March 2009 to March 2012, reduction mammaplasty was performed in 32 patients. Removal of excessivegland was performed by inframammary fold incision. The central gland pedicle and inferior dermal pedicle were formed to ensure enough blood supply for the areola of nipple. The inferior dermal pedicle was fold- ed as implant and using the extremely breast skin to coverthe implant as new breast outer layer. Re- sults After 6- 36 months follow- up for 32 cases, all patients were satisfied with the shape of breast, unobvious scar, and normal areola of nipple. Conclusion The method of reduction mammaplasty using the inferior dermal pedicle with inframammary fold incision technique is an ef- fective therapy for moderate and severe breast hypertrophy, which has the advantage of natural and beautiful breast shape with hidden scar.
