利用金属导丝在螺旋CT定位下进行微创手术治疗脑血肿的临床应用

摘　　要：目的 探讨通过螺旋CT定位微创手术治疗脑血肿的临床应用价值.方法 收集我院2009年1月～2012年12月期间67例脑血肿患者资料.按照三维立体定向原理：两面相交,交线唯一.利用CT扫描同时,用金属导丝标记,使得金属导丝标记平面与扫描平面相交所成的直线决定穿刺方向.结果 67例患者均在24小时内施行手术,66例微创手术成功,成功率为98.5%.25例患者功能基本恢复,日常生活自理,并投入到正常生活和工作中;21例部分功能缺失,生活基本自理;11例生活难以自理,但借助拐杖等能行走;8例需要卧床,不能行走;死亡2例.结论 利用金属导丝螺旋CT定位微创手术治疗高血压脑血肿操作简单、定位准确、效果满意、有效降低脑出血患者的死亡率和致残率,为病人在治疗和康复起到关键的作用.Objective To study the clinical application value of minimally invasive surgery for cerebral he- matoma by spiral CT positioning. Methods 67 cases of patients with cerebral hematoma was collected from January 2009 to December 2012. According to the 3D directional principle, both sides intersect, intersecting lines only. CT scanning with a metal thread tag, makes the straight intersected by metal thread tag plane and scanning plane decide puncture direction. Results 67 cases of patients were implemented surgery within 24 hours, 66 cases of minimally invasive surgery is successful, the success rate is 98.5%. 25 cases of patients has basic function, daily life care, and put into the normal life and work; 21 cases has Partial features loss and basic self- care life; 11 cases is difficult to take care of themselves, but able to walk with crutches ; 8 cases need to stay in bed, unable to walk ; 2 cases died. Conclusion Guide wire in minimally invasiveoperation by spiral CT localization is simple operating, accurate posi- tioning, satisfied effect, and effectively reduce the mortality and morbidity in patients with brain hemorrhage. It also play a key role in the treatment and rehabilitation for the patient.
