腹腔镜与开腹肝切除术治疗肝脏疾病的疗效比较

摘　　要：目的比较腹腔镜肝切除术(laparoscopic hepatectomy,LH)与开腹肝切除术(open hepatectomy,OH)治疗各种肝脏疾病的疗效,探讨LH的可行性。方法比较73例LH组和73例OH组患者术前、术中及术后指标,同时比较这两种术式在不同肝脏疾病中疗效的差异。结果 LH术和OH术在各病种的切口长度、禁食时间、术后疼痛、术后ALT恢复正常时间上差异有统计学意义(P<0.05),在住院费用、并发症发生率上差异无统计学意义(P>0.05),在手术时间、术中出血量、术后血液指标、住院时间方面,不同的病种两种术式的差异有所不同。结论 LH在术中出血、手术时间、术中损伤及术后恢复方面优于OH,该优势在不同肝脏疾病类型间表现不同,LH是肝脏疾病安全、可靠的治疗方法。Objective To explore the feasibility of laparoscopic hepateetomy by comparing the curative effect of laparqscopic hepatectomy and open hepatectomy for liver disease. Methods We compared the data of 73 patients undergoing laparoscopic hepatectomy and 73 patients undergoing open hepatectomy, and the difference in curative effects of them for different liver disea-ses. Results The significant differences between the two groups were noticed in the length of incision, fasting time, postoperative pain, postoperative recovery time of ALT （ P 〈0. 05 ） , but not in hospitalization costs and complication rate （ P 〉 0.05 ）. For differ-ent liver diseases, there was difference in operation time, bleeding volume, postoperative blood index, hospitalization time between laparoscopic hepatectomy and open hepatectomy. Conclusion Lapar0scopic hepatectomy is feasible and safe. Its advantages were differently showed among different types of liver disease. It is better in operation time, bleeding volume, intraoperative injury and postoperative reeovery as compared to open hepateetomy.
