摘 要:目的掌握江苏省江阴市2012年感染性腹泻病的发生、流行特征、变化趋势及病原体变化特点,为制定预防措施和临床治疗提供依据。方法对中国疾病控制信息系统“疾病监测信息报告管理系统”,收集到的江阴市2012年感染性腹泻病例报告信息进行统计分析,并对5家哨点医院感染性腹泻病例的492份样品进行沙门氏菌、志贺氏菌、致泻性大肠杆菌、致泻性弧菌、金黄色葡萄球菌、变形杆菌检测。66份样品进行轮状病毒、札如病毒、诺如病毒、星状病毒和腺病毒检测。结果2012年全市共发生其他感染性腹泻病例1760例,发病率为1.46‰,死亡2例,病死率为1.14‰。性别上男女发病差异无统计学意义(x2=0.32;P〉0.05);在年龄分布上发病人数最多的为40~岁年龄组,占病例总数的17.27%;在职业上以农民为主,占46.31%,在时间分布上以6~8月份和10~11月份为高发。558份腹泻标本,病原检出率为5.56%,肠道致病菌检出率为2.24%,肠道病毒检出率为30.30%;两者间差异有统计学意义(,=92.80;P〈0.005)。在性别上男性的检出率高于女性,差异有统计学意义(x2=4.66;P〈0.05)。感染性腹泻6~10月份以肠道致病菌为主,10月至次年5月份以肠道病毒为主。结论江阴市2012年的感染性腹泻以病毒为主,主要是诺如病毒,细菌性腹泻以副溶血性弧菌为主,初步揭示了全市感染性腹泻病原谱。Objective To understand the prevalence,epidemic features,trend and pathogens of infectious diarrhea diseases in Jiangyin city in 2012, so as to provide basis for clinical treatment and take preventive measures. Methods Infectious diarrhea case report information of Jiangyin city in 2012 was collected from" China information system for disease control" for statistical analysis. Infectious diarrhea cases were collected from 5 sentinel hospitals. In these cases,492 samples were used for the detection of bacteria, including salmonella, shigella, staphylococcus aureus, bacillus proteus, E. coli and vibrioparahaemolyticus. And 66 samples were used for the detection of virus, including rotavirus, sapporo like viruses, norovirus,astrovirus and adenovirus. Results In 2012, 1 760 cases of infectious diarrhea cases were detected, the incidence being 1.46%o. 2 deaths, the fatality rate was 1. 14‰. The difference between male and female was not statistically significant (X2 =0. 32,P 〉0. 05) ; The age distribution showed that the majority was in the forty year old-age group,accounting for 17. 27% of the total number of cases; farmers were the main victims (46.31%) ,Seasonal distribution showed that June-August and October-November were the two peaks. From 558 diarrhea specimens, pathogen detection rate was 5.56%. Intestinal pathogenic bacteria detection rate was 2.24% ,the intestinal virus detection rate was 30.30% ; The difference was statistically significant (X2 = 92. 80, P 〈 0. 005). The detection rate of male was higher than that in female, the difference was statistically significant (X2 = 4. 66 ,P 〈 0. 05 ). The pathogen of infectious diarrhea,from June to October was mainly caused by bacteria,while from October to the following May,mainly caused by virus. Conclusion Noroviruswas the main pathogen causing infectious diarrhea in Jiangyin city in 2012, while vibrio parahaemolyticus was the main pathogen causing infectious diarrhea in Jiangyin city in 2012.
