摘 要:目的 探讨蛋白酶抑制剂乌司他丁对失血性休克后急性肺损伤及HMGB1水平的影响.方法 将54只SD雄性大鼠随即分为三组:正常对照组、生理盐水+休克组、乌司他丁+休克组.大鼠提前5分钟经股静脉注射给药后放血休克90分钟,分别在造模完成后3小时、6小时、12小时处死测量其动脉血PaCO2、PaO2、血乳酸、干湿重比法测量肺组织含水率、western blot法检测肺组织HMGB1表达,HE染色观察病理变化.结果 休克后动脉血PCO2、血乳酸、肺组织含水率及HMGB1表达均明显增高,PaO2明显降低,肺组织间质水肿、炎性细胞浸润明显,提示急性肺损伤的发生,而乌司他丁+休克组的动脉血PaCO2、PaO2、血乳酸、干湿重比法测量肺组织含水率及HMGB1较生理盐水+休克组明显改善.结论 乌司他丁减轻肺部炎症反应抑制失血性休克所致的急性肺损伤从而改善肺功能,其机制可能与降低肺组织HMGB1表达有关.Objective To observe the effect of Ulinastatin on the acute lung injury (ALI) and the expression of high mobility group box 1 protein (HMGB1) in hemorrhagic shock rat lung.Methods A total of 54 healthy male SD rats were randomly divided into the following 3 groups:normal control,normal saline (NS)+shock,Ulinastatin+shock.Rats were subjected to administration of NS or Ulinastatin before the 90 minshock,followed by execution respectively at 3h,6h and 12h to test the PaCO2,PaO2 and level of blood lactate,the rate of water content and the expression of HMGB1 of the lung were measured respectively by the wet/dry weight ratio method and western blot.Histopathologic changes were observed by hematoxylin-eosin staining.Results Increased PaCO2,lactate level,the rate of water content and the expression of HMGB1 were found,decreased PaO2 and massive inflammatory cell infiltration were observed in NS+shock group indicating ALI compared with those in normal control group.In Ulinastatin+shock group,these alterations were partially inhibited.Conclusion The high expression of HMGB1 and the inflammatory reaction in ALI rat induced by hemorrhagic shock can be attenuated by Ulinastatin,followed by lung function improvement.
