强化提肛肌训练对经尿道前列腺剜除术后尿失禁改善的效果观察

摘　　要：目的 探讨强化提肛肌训练对促进经尿道前列腺剜除术后尿失禁恢复的效果.方法 将2011年10月至2012年10月在我院行前列腺增生经尿道前列腺剜除术的178例患者随机分为实验组89例和对照组89例.实验组入院当天开始给予严格的提肛肌训练,并持续到术后尿失禁缓解为止；对照组术后拔除尿管后才给予提肛肌训练,持续到尿失禁缓解为止.比较两组术后尿失禁发生率,尿失禁严重程度,尿控恢复时间.结果 实验组术后尿失禁发生率为32.6％,低于对照组91％,P＜0.001;但两组尿失禁严重程度比较差异并无统计学意义；实验组尿失禁中位恢复时间（＜1周）显著短于对照组（2周）,P＜ 0.001.结论 术前强化提肛肌训练对降低前列腺剜除术后尿失禁的发生率,缩短术后尿失禁恢复时间起重要作用.Objective To observe the effect of intensive levator ani muscle training for early recovery of urinary continence following transurethral enunciative resection of prostate （TUERP）.Methods We randomly assigned 178 patients undergoing TUERP from October 2011 to October 2012 in our institute into 2 groups.The study group included 89 patients starting levator ani muscle training from the admission day till the day postoperative incontinence was cured; the control group also included 89 patients who were taught to undertake levator ani muscle training from the day of catheter removal till the day postoperative incontinence was cured.The incidence of incontinence,the severity of incontinence and the duration of incontinence were compared.Results The incidence of incontinence （32.6％） in the study group was significantly lower than that in the control group （91％,P ＜ 0.001）,while the severity of incontinence between 2 groups did not manifest significant difference （P--0.125）.Patients in the study group underwent earlier recovery of urinary incontinence with a median recovery time of less than 1 week,compared to that in the control group of 2 weeks （P ＜ 0.001）.Conclusion Intensive levator ani muscle training help reduce incontinence rate and early recovery of urinary continence following TUERP.
