类风湿关节炎患者治疗依从性及其影响因素分析

摘　　要：目的探讨类风湿关节炎(RA)患者的治疗依从性并分析其影响因素。方法采用自行设计的类风湿关节炎患者治疗依从性调查表对106例类风湿关节炎患者进行调查。结果影响类风湿关节炎患者治疗依从性的因素包括性别、婚姻、病程、家庭收入和文化程度等,但与年龄无关;进一步分析显示,疾病相关知识缺乏、对病情严重度的认识不足、经济状况差以及疾病与工作/生活的冲突是造成RA患者依从性差的主要因素。多因素回归分析显示经济原因、不知是否需要长期坚持服药、治疗方案太复杂以及对疾病没有足够了解等是RA患者无法遵医嘱的主要原因,而畏惧药物副作用并不是RA患者无法遵医嘱的原因。结论医护人员应重视类风湿关节炎患者的健康教育,使患者树立对疾病恢复及生活的信心,提高治疗依从性,从而减轻患者的痛苦,降低患者关节变形的概率,提高生活质量。Objective To explore the therapeutic compliance of patients with rheumatoid arthritis and analyze the influential factors. Methods The self - designed questionnaire on rheumatic disease was used to survey the therapeutic compliance of 106 patients with rheumatoid arthritis. Results Factors influencing the therapeutic compliance of patients with rheumatoid arthritis were various, including gender, marital status, disease duration, the income of family and education level and so on, but excluding age; further analysis showed that some factors such as lack of knowledge on disease, inadequate understanding of the severity of disease, poor economic condi- tions and the conflict between work and life and were the main factors of poor therapeutic compli- ance in patients with rheumatoid arthritis. A multivariate logistic regression analysis showed that e- conomic reasons, long- term adherence to medication or not, the complication of options of treat- ment and inadequate understanding of the disease were the main reasons of poor therapeutic compli- ance, but excluding fear of adverse reaction of drugs. Conclusion The medical staffs should pay more attention to enhance the health education of patients, alleviate the suffering of patients, im- prove the quality of life, reduce the probability of joint deformity and reinforce the confidence of pa- tients.
