摘 要:目的探讨研究血管性血友病因子(vWF)水平在2型糖尿病患者(2TDM)的中变化及其与冠心病的关系。方法选取2010年1月~2013年1月内蒙古自治区人民医院收治的160例老年2型糖尿病患者为研究对象.根据2型糖尿病的诊断标准分为2型糖尿病合并冠心病组(2TDM+CHD组)与单纯2型糖尿病组(2TDM组),每组各80例,同期选择健康体检的非糖尿病患者80例为对照组,抽取空腹血测定血管性血友病因子(vWF)、血小板a颗粒膜糖蛋白(CD62P)、溶酶体膜蛋白(CD63)与血管性血友病因子裂解酶(vWF—cp)的水平,分析其相关性。结果2TDM±CHD组患者CD62P、CD63、vWF水平分别为(11.3±6.4)%、(13.5±7.2)%、(166.9±18.7)%,显著高于2TDM组与对照组,差异有统计学意义(P〈0.05),而vWF—cp活性[(58.7±17.5)%]显著低于2TDM组与对照组(P〈0.05);2TDM组患者CD62P、CD63、vWF水平分别为(7.3±3.5)%、(8.3±4.2)%、(142.3±20.8)%,显著高于对照组(P〈0.05),而vWF—cp活性[(65.4±18.1)%1显著低于对照组(P〈0.05),直线相关分析表明冠脉病变积分与vWF水平呈正相关(r=0.732,P〈0.05)。结论血浆vWF水平与2型糖尿病发生、发展、内皮损伤有关,且与2型糖尿病冠心病的发生存在一定关联,加强血浆vWF检测在糖尿病发病及冠心病发展的监测中具有一定的临床价值。Objective To explore and study the change of von Willebrand factor (vWF) levels to patients with type 2 dia-betes mellitus (2TDM) and its relationship with coronary heart disease (CHD). Methods 160 cases of elderly patients with type 2 diabetes from January 2010 to January 2013 in Inner Mongolia People's Hospital were selected as the research object. According to the diagnostic criteria for type 2 diabetes, they were divided into the group of type 2 diabetes mellitus combined with coronary heart disease (2TDM+CHD group) and the group of pure type 2 diabetes mellitus (2TDM group), with 80 cases in each group. 80 cases of non-diabetic patients with physical examination at the same period were selected as the control group, the level of von Willebrand factor (vWF), platelet granule membrane glyeoprotein a (CD62P), lysosome membrane pro-tein (CD63) and yon Willebrand factor lyase (vWF-ep) were determined by extracting the fasting blood and their correlation were analyzed. Results The CD62P, CD63, vWF levels of patients in 2TDM+CHD group were (11.3±6.4)%, (13.5±7.2)%, (166.9±18.7)% respectively, which were significantly higher than those in 2TDM group and control group (P 〈 0.05), while vWF-cp activity was significantly lower than that in 2TDM group and control group (P 〈 0.05); the CD62P, CD63, vWF level of pa-tients in 2TDM group were (7.3±3.5)%, (8.3±4.2)%, (142.3±20.8)% respectively, which were significantly higher thanthose in normal control group (P〈 0. 05), while the vWF-cp activity [(65.4±18.1)%] was significantly lower than that in control group (P 〈 0.05), the linear correlation analysis showed that coronary disease score and vWF levels were positive correlated (r = 0.732, P 〈 0.05). Conclusion Plasma vWF level is related to the occurrence, development and endothelial injury of type 2 diabetes mellitus, and there is a certain correlation with the incidence of type 2 diabetes mellitus with coronary heart
