摘  要:目的探讨高频彩超在乳腺癌筛查中对早发现、早诊断、早治疗(“三早”)的应用价值与前景及随诊意义。方法应用高频彩超对2011年1月~2013年7月成都市龙泉驿区第一人民医院在门诊、体检中心及仁爱社区服务中心的适龄女性10039例进行乳腺癌筛查,观察其声像图特点,对病变者进行BI—RADS及血流Adler分级;对可疑恶性肿块选择性超声造影。对要求定期复查的可疑患者每3N6个月进行复检随访。结果10039例中共检出乳腺疾病1921例,占19.13%。1921例行BI—RADS分级:其中1级3例,占0.16%;2级832例,占43.31%;3级910例,占47.37%;4级150例,占7.81%;5级26例,占1.35%。对34例肿块进行了Adler血流分级:其中3级4例,占11.77%;4级20例,占58.82%;5级10例,占29.41%。对怀疑恶性肿块选择性超声造影10例,富血管肿块(快进快出)9例,乏血管肿块1例(慢进慢出)。1921例乳腺疾病手术病理证实116例,良性肿块73例,占3.80%(73/1921);恶性肿块43例占2.24%(43/1921)。术前超声明确倾向恶性肿块26例,手术结果证实25例,超声诊断符合率96.15%(25,26)。结论高频彩超在社区乳腺癌筛查中有利于早发现、早诊断、早治疗,且简便、易行、安全,适合大规模体检,具有重要的应用价值与广阔前景。Objective To study the value, prospect and follow-up meaning of high-frequency color ultrasonic in breast cancer screening of early finding, early diagnosis and early treatment ("three early"). Methods The characteristics of high-frequency color ultrasonic imaging were analyzed retrospectively in 10 039 female patients in outpatient, physical examination and Re-nai community service in the First People's Hospital of Longquanyi District in Chengdu City from January 2011 to July 2013, who were classified by BI-RADS and the Adler grade of blood flow; and selectively contrast-enhanced ultrasound was per- formed in suspicious malignant tumor. Those suspected cases were required follow-up of recheck regularly in every three to six months. Results In 10 039 cases, 1921 cases of breast disease were found, the percent was 19.13%. By BI-RADS, among the 1921 cases, 3 cases (0.16%) of level 1, 832 cases (43.31%) of level 2, 910 cases (47.37%) of level 3, 150 cases (7.81%) of level 4, 26 cases (1.35%) of level 5 were found. By Adler grade of blood, among the 34 cases of tumo, 4 cases (11.77%) of level 3, 20 cases (58.82%) of level 4, 10 cases (29.41%) of level 5 were found. 10 cases were selected for contrast-enhanced ultrasound of suspicious malignant tumor, including 9 cases of rich blood vessels in tumors (quick in and fast out) and 1 case of lack of tumor blood vessels in tumor (slow in and slow out). In 1921 cases of breast disease, 116 cases were confirmed by surgical pathology; including 73 cases (3.80%) of benign breast tumor, 43 cases (2.24%) of malignant tumor. Before the surgeries, 26 cases of malignant tumor were diagnosed by ultrasound clearly; 25 cases were proved in surgery. The ultrasound diagnosing coincidence rate was 96.15% (25/26). Conclusion High-frequency color ultrasonic in breast cancer screening is a simple, feasible, safe method for large-scale medical examination and beneficial to early finding, early diagno-sis and early treatment, which
