异丙酚对呼吸机所致肺损伤大鼠肺组织TGF-β1/Smad2信号通路的影响

摘  要:目的 评价不同时点异丙酚对呼吸机所致肺损伤和组织转化生长因子(TGF)-β1-Smad2信号通路的影响.方法 健康雄性Wistar大鼠50只,体质量260~300 g,采用随机数字表法,随机分为五组(每组10只):常规潮气量通气组(A组,VT=8 mL/kg)、大潮气量通气组(B组,VT =30 mL/kg)、异丙酚不同时点给药组(C~E组).C组、D组和E组分别于给予大潮气量通气前1h、给予大潮气量通气前即刻和给予大潮气量通气后1h时静脉注射异丙酚5 mg/kg,随后以10 mg/(kg·h)速率输注至机械通气4h结束.收集肺组织标本,测定肺泡灌洗液(BALF)蛋白、肺通透指数(LPI)及肺组织湿/干质量比(W/D);测定TGF-β1 mRNa和Smad2表达.结果 与A组比较,B~E组W/D、LPI增加,TGF-β1 mRNa和Smad2表达上调(P〈0.05);与B组比较,C组、D组W/D、LPI降低,TGF-β1 mRNa和Smad2表达下调(P〈0.05).结论 异丙酚早期给药可减轻大鼠呼吸机所致肺损伤,其机制与抑制TGF-31/Smad2通路激活有关.Objective To investigate the effects of propofol treatments on ventilator - induced lung injury and the expression of transforming growth factor (TGF) - β1 - Smad2 signal pathway of rats. Methods Fifty male Wistar rats (260 -300 g, 7 -8 weeks old) were randomly divided into 5 groups: standard ventilation group (group A, VT = 8 mL/kg), high -tidal volume ventilation group( group B, received high- tidal volume ventilation VT = 30 mL/kg), propofol treated groups (group C, D and E, received propofol 5 mg/kg 1 h before, immediately after and 1 h after high - tidal volume ventilation administration, respectively ), and then the treated groups were followed by infusion of propofol at 10 mg/( kg·h)until 4 h after high - tidal volume ventilation. Then the animals were sacrificed, and the lung tissues were immediately removed for calculation of wet/dry lung weight ratio (W/D) and lung permeability index (LPI). Lung pathologic changes were observed under light microscope, and the expression of TGF - β1 mRNA and Smad2 protein in lung tissue was determined. Results The four experiment groups (B -E) all showed significant increase in W/D, LPI and the expression of TGF -β1 mRNA and Smad2 protein when compared with tile control group( P 〈0.05 ). When compared with group B, both groups C and D showed obviously attenuated increase in W/D, LPI and expression of TGF - β1 mRNA and Smad2 (P 〈 0.05). Conclusion Early propofol administration may attenuate ventilator - induced lung injury, which may be related to the inhibition of TGF - β1 - smad2 signal pathway.
