摘  要:目的探讨二维超声斑点追踪成像技术评价原发性高血压患者左心室整体和局部收缩功能的临床价值。方法获取各组心尖二腔、三腔及四腔心切面二维动态图像,测定左室各节段收缩期纵向峰值应变,并计算左室心尖段、中间段及基底段水平各节段收缩期平均峰值应变作为左室整体应变。结果与对照组比较,高血压左室正常构型组及左室重构组各节段收缩期纵向峰值应变显著降低(P<0.05或P<0.01),与对照组比较,高血压各组心尖水平、中间段水平及基底水平各节段纵向收缩期平均应变值及左室整体应变值显著降低(P<0.01)。结论超声斑点追踪成像技术能定量不同左室构型高血压患者心肌纵向应变,可以早期准确地评价原发性高血压患者左室局部和整体心肌收缩功能。[Objective] To investigate the clinical value of two-dimensional speckle tracking imaging (STI) in assessing the left ventricular (LV) regional and global systolic function in patients with essential hypertension (EH). [Methods] High frame rate images were recorded from the LV apical two-chamber view, three chamber view and four-chamber view. Acquired the peak systolic strain of each segment, calculated the average strain value of three consecutive cardiac cycles as the peak systolic strain. The average strain values of the three LV long axis levels and three LV short axis levels were calculated as the LV global LS. [Results] Compared with those in control group, the peak systolic longitudinal strains of each segment in EH patients decreased significantly (P 〈0.05 or P〈0.01),and average peak strain in the long axis at apical, middle and basal levels and mean peak strain of average segments were decreased too (all P〈0.01). [Conclusion] Twodimensional speckle tracking can assess left ventricular regional and global systolic function early in essential hypertension patients.
