低分子右旋糖酐注射液对颈动脉内膜剥脱术后患者凝血指标的改善

摘　　要：目的:通过对颈动脉内膜剥脱术(carotid endarterectomy,CEA)后血栓弹力图(thrombelastography,TEG)指标,评价术后早期应用低分子右旋糖酐对围术期凝血功能的影响。方法:入选2012年6月至2013年1月入住我院SICU的行颈动脉内膜剥脱术患者60例,比较患者术后常规应用低分子右旋糖酐前后的血栓弹力图各参数的回顾性分析。结果:研究显示CEA术后患者应用低分子右旋糖酐未出现脑栓塞、急性脑梗死,出血等并发症,血栓弹力图显示MA值术后(55.79±6.20)mm较术前(60.56±4.23)mm下降,而CI值术后(-1.00±0.44)较术前(0.27±0.48)亦下降,差异具有统计学意义(P<0.05),而手术前后血常规检查显示血小板差异无统计学意义(P>0.05)。结论:低分子右旋糖酐对CEA术后预防栓塞性并发症是一种安全、有效和合理的治疗方式。Objective：To study the blood coagulation effect of Dextran 40 on the patients with carotid endarterectomy （CEA）by thrombelastography（TEG）. Methods ：A total of 60 patients with carotid endarterecto- my treated with Dextran 40 were determined by TEG, Comparing the results before and after treatment to evalu- ate its coagulation function. Results： The patients with CEA treated with Dextran 40 were free of complications perioperatively. There was a significant difference in maximal amplitude （MA） between pre-operation（60. 56±4. 23 ）mm and post-operation [ （ 55.79±6. 20） ram, P 〉 0. 05 ], while the index of CI post-operation （ 1.00 ± 0.44） were less than pre-operation [ （ 0. 27 ± 0.48 ）, P 〈 0. 05 ]. Conclusion： Dextran 40 is a safe and useful treatment for CEA patients to preclude carotid intraluminal thrombosis postoperatively.
