老年患者脂肪肝特点及其与血尿酸水平的相关性

摘　　要：目的探讨老年患者脂肪性肝病(fatty liver disease,FLD)的特点及其与血尿酸水平等代谢指标的相关性。方法选择住院患者268例,年龄69～88(78.5±10.2)岁,分为非脂肪肝组和脂肪肝组。分析其脂肪肝的特点及与血尿酸水平等代谢指标的相关性。结果老年患者以非酒精性轻中度脂肪肝为主要特点,脂肪肝组的血尿酸、谷丙转氨酶、血糖、体质量指数、低密度脂蛋白胆固醇、三酰甘油均高于非脂肪肝组;血红蛋白低于非脂肪肝组;脂肪肝组冠心病、糖尿病、脑卒中的发病率高;差异均有统计学意义(均P<0.05)。Logistic回归分析显示,冠心病、脑卒中、吸烟、GPT高、尿酸高、血红蛋白低是脂肪肝的危险因素,均呈正相关(均P<0.05)。其他指标未进入方程。结论老年患者以非酒精性轻中度脂肪肝为主要特点,脂肪肝患者的尿酸、血脂、体质量指数高,冠心病、糖尿病、脑卒中的发病率高,血红蛋白低。对老年患者除控制冠心病的常见危险因素外,还应重视脂肪肝、高尿酸血症、低血红蛋白的综合治疗。Objective To observe the features and association between non alcoholic fatty liver and serum uric acid （UA） and other metabolic dysfuntions in senile patients. Methods A total of 268 patients, with an average age of （78.5？10.2）years, were divided into two groups： fatty liver （FLD） group and non fatty liver group. The association of UA and other metabolic dysfuntions between the two groups was analyzed. Results The levels of UA with FLD（408.32？65.43）mmol/L were significantly higher in fatty liver group than those non fatty liver group（329.79？87.99）mmol/L. （P〈0.05）. Levels of coronary heart disease with FLD（66.2%）was significantly higher than those in non fatty liver group（39.1%）（P〈0.05）. Patients with FLD had higher body mass index （BMI）, higher glutamate-pyruvate transaminase （GPT）, higher blood glucose, higher triglyeride han patients with non fatty liver （all P〈0.05）, lower hemoglobin than patients with non fatty liver （P〈0.05）. Logistic regression with FLD as the dependent variables showed that coronary heart disease was positively correlated with FLD, smoking, high UA, GPT and low hemoglobin （all P〈0.05）. Conclusions Coronary heart disease is closely associated with FLD. Patients with FLD is correlated positively to UA, diabetes mellitus, high triglyeride and coronary heart disease and low hemoglobin.
