有创中心动脉压与外周动脉压差及其与冠状动脉病变程度的关系

摘　　要：目的观察中心动脉压与外周动脉压之间的差异及其与冠状动脉粥样硬化病变程度之间的关系。方法选取同期行冠状动脉造影的患者322例，其中男218例，女104例，年龄40～82（59．61±9．01）岁。冠状动脉造影时同时测定中心动脉压及外周动脉压，根据冠状动脉造影结果对冠状动脉病变严重程度进行分组。观察各组间中心-外周脉压之间的差值（简称中-夕r脉压差）同冠状动脉粥样硬化病变程度之间的关系。结果冠心病组中-外脉压差明显高于非冠心病组[（29．47±16．71）mmHg比（19．97±7．99）mmHg，P〈0．05]。在冠状动脉狭窄程度、冠状动脉病变支数、冠状动脉Gensini积分的分组中，随着冠状动脉粥样硬化程度的增加，中-外脉压差增大，各组两两间比较差异均有统计学意义（P〈0．05）。结论中-外脉压差同冠状动脉粥样硬化病变程度有明显的相关性，可预测冠状动脉粥样硬化病变的程度。Objective To investigate the differences of pulse pressure of central aortic pressure and peripheral aortic pressure in order to study the relationship between the differences of pulse pressure and the extent of coronary atherosclerotic lesions. Methods A total of 322 patients who undergent coronary angiography were enrolled in the study. Among them, there were 218 males and 104 females, and the mean age was 59. 61±9. 01 years. All patients were grouped according the degrees of their coronary artery stenosis, central and peripheral aortic pressures were measured through the catheter while coronary angiography performing. The relationship between the pulse pressure differences of central and peripheral were then studied and the extent of coronary artery stenosis. Results The difference in pulse pressure （PP） was higher in coronary artery diseases group than that of non-coronary artery diseases group, and there was a significant difference between the two groups [ （29.47± 16. 71 ） mm Hg vs. （ 19. 97±7.99 ） mm Hg, P 〈 0. 05 ]. Among the groups with varied degrees of coronary artery stenosis, numbers of vessel disease and different Gensini scores of coronary artery, the degrees of coronary atherosclerotic lesion increased as the differences of pulse pressure between central and peripheral increased, all P 〈 0. 05. Conclusions The differences of pulse pressure of central and peripheral were positively correlated with the coronary atherosclerotic lesions.
