应变及应变率成像评价酒精性心肌病右心室纵向心肌局部舒缩功能的价值

摘　　要：目的探讨应变及应变率成像技术评价酒精性心肌病(ACM)右心室纵向心肌局部舒缩功能的价值。方法酒精性心肌病患者30例(ACM组)和健康志愿者30例(正常对照组),分别检测两组常规超声参数、应变及应变率参数。应变及应变率参数通过心尖四腔切面右心室游离壁3个节段的应变及应变率曲线获取,并测量每一曲线上的收缩期和舒张期峰值。收缩期参数包括:右心室游离壁基底段、中间段和心尖段收缩期应变峰值(S)、收缩期应变率峰值(SRs);舒张期参数包括:舒张早期应变率峰值(SRe)和舒张晚期应变率峰值(SRa)。结果 ACM组右心室前后径较正常对照组显著增厚,差异有统计学意义(P﹤0.05),但右心室射血分数略微减低,差异无统计学意义。ACM组右心室游离壁基底段、中间段和心尖段S、SRs、SRe和SRa与对照组比较均显著减低,差异均有统计学意义(P﹤0.05)。结论应变及应变率成像可作为评价ACM右心室纵向心肌局部舒缩功能的一种无创的、有效的新方法。Objective To evaluate the regional fight ventricular longitudinal systolic and diastolic function in patients with alcoholic cardiomyopathy （ACM） by strain and strain rate imaging. Methods Thirty patients with ACM and 30 healthy adults （control group） were examined. The conventional echocardiographic parameters, strain and strain rate imaging parameters were obtained in the two groups. Strain and strain rate imaging parameters were recorded from the strain and strain rate curves, which were obtained from basal to apical of right ventricular free wall in apical four-chamber by strain and strain rate imaging, the parameters of peak longitudinal systolic strain （S） ,systolic strain rate （SRs） ,early diastolic strain rate （SRe） and late diastolic strain rate （SRa） were included.Results Compared with the control group, RVEDd were significantly increased in ACM group （P 〈 0.05）,but there was no significant difference in RVEF between the two groups. The S,SRs,SRe and SRa of all right ventricular free wall segments obtained by strain and strain rate imaging were significantly decreased in ACM group （P 〈 0.05）. Conclusion Strain and strain rate imaging provide an effective and non-invasive tool to assess the regional right ventricular longitudinal systolic and diastolic function in patients with ACM,which have important clinical value.
