摘  要:目的探讨肝细胞生长因子(hepatocyte growth factor,HGF)对阿霉素肾病大鼠足细胞人肾蛋白(nephrin)表达的影响。方法雄性清洁级SD大鼠114只,分为对照组(42只)、模型组(42只)和干预组(30只),对照组尾静脉一次性注射等体积的生理盐水溶液;实验组和干预组大鼠尾静脉内一次性注射阿霉素溶液建立阿霉素肾病模型,干预组于造模后第3周开始给予HGF腹腔注射直至实验结束;对照组与模型组造模成功后第1周末、2周末、3周末、4周末、6周末、8周末和12周末处死动物。干预组于注射HGF 1周后开始,即造模成功后3周末、4周末、6周末、8周末和12周末处死动物,处死前1天留尿液检测24小时尿蛋白,处死后取肾组织进行免疫荧光染色,用激光共聚焦显微镜检测肾小球足细胞中人肾蛋白的荧光表达强度。结果与对照组比较,模型组尿蛋白明显增加,造模成功后第1周末、2周末、3周末、4周末、6周末、8周末和12周尿蛋白依次为(854.3±77.0)mg/L vs(330.6±33.1)mg/L,(814.8±158.6)mg/L vs(343.4±77.0)mg/L,(795.0±242.3)mg/L vs(415.6±113.5)mg/L,(703.5±137.3)mg/L vs(355.1±84.8)mg/L,(621.7±241.2)mg/L vs(350.4±61.8)mg/L,(751.4±389.9)mg/L vs(369.3±62.7)mg/L,(725.4±170.2)mg/L vs(435.2±140.9)mg/L(P<0.01),给予HGF干预后,尿蛋白明显减少(P<0.01);与对照组比较,模型组大鼠人肾蛋白暴露明显增多(P<0.01);给予HGF干预后人肾蛋白暴露较模型组明显减少(P<0.01)。结论阿霉素肾病大鼠尿蛋白明显增加,人肾蛋白暴露呈先升后降的变化趋势;HGF可减轻急性期足细胞损伤,对足细胞具有保护作用,减轻大鼠阿霉素肾病肾组织的病理损害,减少尿蛋白。Objective To observe the effect of hepatocyte growth factor(HGF) on foot cell nephrin expression in adriamycin nephrosis rats. Methods 114 male SD rats of clean grade were divided into the control group ( n =42), model group ( n =42) and intervention group ( n =30),control group injected intravenously through tail vein once an equal volume of saline solution; the experimental group and the intervention group injected intravenously through tail vein with adriamycin solution to establish a one-time cyclosporine nephropathy model,the intervention group for third weeks after modeling was given by intraperitoneal injection of HGF until the end of the experiment;the rats of control group and the model group sacrificed after first, 2,3,4,6,8 and 12 weeks after successful animal modeling. The rats of intervention group sacrificed in the injection of HGF after the first week,namely after 3 weeks,4 weeks,6 weeks,8 weeks and 12 weeks after successful animal modeling, the day before sacrifice, 24 hour urinary protein, renal tissue were detected by immunofluorescence staining,the expression intensity of nephrin was tested by using fluorescence confocal microscopy detection of the glomeruli in podocytes. Results Compared with the control group, urine protein in model group increased significantly, urine protein after first, 2,3,4,6, 8 and 12 weeks after successful animal modeling, respectively (854.3±77.0) mg/L vs (330.6±33.1) mg/L, (814.8± 158.6) mg/L vs (343.4± 77.0) mg/L, (795.0± 242.3) mg/L vs (415.6±113.5) mg/L,(703.5±137.3) mg/L vs (355.1±84.8) mg/L,(621.7±241.2) mg/L vs (350.4±61.8) mg/L,(751.4±389.9) mg/L vs (369.3± 62.7) rag/L,(725.4±170.2) mg/L vs (435.2±140.9) mg/L,respectively( P 〈0.01), given the intervention of HGF, urinary protein significantly reduced (P 〈0.01).
