经阴道二维联合彩色及多普勒超声对流产后宫腔内残留物的诊断价值

摘　　要：目的:探讨经阴道二维超声联合彩色脉冲多普勒在流产后宫腔内残留物的诊断价值。材料与方法:对220例流产后宫腔内残留的患者行阴道超声CDFI及PW检查,观察宫腔及血流情况,测量其血流参数:Vmax(收缩期峰值速度)、Vmin(舒张末期速度)、RI(阻力指数),并对其总结分析。结果:220例患者中分为3种类型,I.188例(85.4%)表现为宫腔内残留物,显示宫腔内不均质团状回声,其中的180例(95.7%)检测到滋养层血流,RI<0.5,II.27例(12.3%)表现为子宫内膜回声不均,局部略增厚,其中的15例(55.6%)探及类滋养层血流,RI<0.5,III.5例(2.3%)宫腔线清晰,CDFI:未探及明显血流信号。结论:阴道超声联合CDFI及PW对流产后宫腔内残留物具有很好的诊断价值,是临床首选的检查方法。Purpose： to explore the value of transvaginal color Doppler ultrasound in diagnosis of residual substance in uterus cavity after abortion. Materials and Methods： A total of 220 patients with uterine residue after drug abortion or clear palace operation were enrolled in the study , then they were observed by Transvaginal CDFI and PW Doppler ultrasound, and the blood flow parameters were measured , including systolic peak velocity （Vmax） , end-diastolic velocity （Vmin）and resistance index （RI）. Results： There were divided into three types ： typel including 188 cases（85.4% ） , there with re- sidual substance in uterus cavity, the echo in uterine cavity was not uniform, 180of them （the display rate of 95.7% ）detec- ted trophoblastic blood flow signal with the color Doppler flow imaging , RI 〈0. 5. type 2 including 27 cases（ 12. 3% ） , the echo of endometium was not uniform, CDFI detected trophoblast cells blood flow in 15 cases with the display rate of 55.6% , RI 〈0. 5. type 3 including 5 cases （2. 3% ） , the 5 patients with normal uterus cavity , all them can not be detected tropho- blastic blood flow signal. Conclusion ： transvaginal-CDFI and PW has high accuracy in diagnosis of residual substance in ute- rus cavity after abortion. It is the first choice in clinical diagnosis.
