血源性组织因子和D-二聚体在老年患者动静脉血栓中的研究

摘　　要：目的研究血源性组织因子(TF)的表达和D-二聚体与老年患者下肢动静脉血栓的关系。方法选取100例经下肢动静脉造影确诊为动静脉血栓的老年患者,分为动脉血栓组50例,静脉血栓组50例;另选择同期健康体检者50例为对照组。检测3组血浆D-二聚体值和TF水平,同时采用半定量RT-PCR法检测3组外周血细胞TF mRNA表达水平,并进行比较。结果与对照组比较,动脉血栓组和静脉血栓组D-二聚体、TF和TF mRNA表达明显升高,差异有统计学意义(P<0.05);与静脉血栓组比较,动脉血栓组D-二聚体、TF和TF mRNA虽有升高趋势,但差异无统计学意义(P>0.05)。相关分析显示,动脉血栓组和静脉血栓组TF mRNA与TF呈正相关(r=0.91,r=0.94,P<0.05)。结论血细胞中TF的表达是外周血TF的主要来源,在老年人动静脉血栓启动、形成、延续阶段起重要作用。Objective To study the relation between blood-borne tissue factor （TF） expression and D-dimer in elderly patients with lower extremity arteriovenous thrombosis. Methods One hundred elderly patients with lower extremity arteriovenous thrombosis established by angiography were divided into arterial thrombosis group （n= 50） and venous thrombosis group （n= 50）, and 50 subjects undergoing health check served as a control group. Their serum D-dimer and TF levels were measured. Expression of TF mRNA in peripheral blood cells was detected by semi-quantitative RT-PCR. Results The expression levels of D-dimer, TF and TF mRNA were significantly higher in arterial thrombosis group and vein thrombosis group than in control group （P〈0.05）. No significant difference was found in expression levels of D-dimer, TF and TF mRNA between arterial thrombosis group and veinous thrombosis group although they tended to increase in arterial thrombosis group （P〉0.05）. Correlation analysis showed that the serum TF mRNA level was positively related with the serum TF level in arterial thrombosis group and veinous thrombo- sis group （r=0.91,r=0.94,P〈0.05）. Conclusion Expression of TF in peripheral blood cells is the main source of peripheral blood-borne TF and plays an important role in initiation,formation and progression of arteriovenous thrombosis in elderly patients with lower extremity arteriovenous thrombosis.
