严季澜辨治高血压病证型分析和用药规律研究

摘　　要：目的挖掘和总结严季澜教授辨治高血压病临证经验。方法将严季澜教授诊治的109例高血压病患者详细病历资料录入数据库,对症状、药物等中医药名词按照国家或行业标准进行规范化处理,运用Excel数据透视表功能对一般资料、证型以及用药等进行统计分析。结果 109例高血压病患者临床可见单证型、双证型及三证型,不同年龄、性别表现出的证型存在差异性,根据统计用药情况归纳出基础方———调肝理血汤,针对不同证型尚有加减用药之法。结论高血压病病机以气阴两虚为本,内热(火)、阳亢、痰浊(或痰热)、瘀血为标,病位以肝肾为主,与心脾(胃)关系密切,涉及脑。严教授针对高血压病的病机特点擅用自拟调肝理血汤加减治疗。Objective To explore and summarize Professor YAN Ji-lan＇ s clinical experience of syndrome differentiation on hypertension. Methods The detailed information of medical history of one hundred and nine patients that were diagnosed and treated by Professor YAN was put into the database. TCM words such as symptoms and herbs were normalized according to national or professional standards. Excel PivotTable was applied to statistically analyze general data, disease pattern and prescriptions. Results Among 109 cases of hypertension, there were pattern of single symptom, pattern of dual symptoms and pattern of three symptoms in clinic; patterns that were presented by different ages and genders were different; by included prescription information, a basic prescription, Tiaogan Lixue Decoction, was summarized. Conclusion With deficiency of both qi and yin as root and inner heat, hyperactivity of yang, phlegm and blood stasis as manifestations, pathogenesis of hypertension is located in liver and kidney and closely related with heart and spleen （stomach）, also involving brain. Aiming at pathogenesis characteristics of hypertension, Professor YAN is good at self-made Tiaogan Lixue Decoction to perform treatment.
