摘 要:目的 检测分析反复呼吸道感染(recurrent respiratory tracti infection,RRTI)患儿血清IL-17和IL-22的表达特点,并探讨它们在RRTI发病机制中的意义.方法 选取滨州医学院附属医院2012年10月~2013年3月期间30例RRTI就诊患儿,分别在治疗前、恢复期采集外周静脉血,同时选取健康体检儿童30例为对照组,应用酶联免疫吸附实验(ELISA)分别检测血清中IL-17和IL-22水平,将患儿组治疗前、恢复期及对照组血清IL-17和IL-22测定数据进行两两比较,作相关分析.结果 ①RRTI患儿治疗前、恢复期及对照组血中IL-17水平分别为24.13±8.80 pg/ml,14.37±8.22 pg/ml和12.07±5.27 pg/ml,治疗前与对照组、恢复期相比,差异均有统计学意义(t=4.281~6.4432,P值均〈0.01);IL-22在治疗前、恢复期及对照组水平分别为76.64±45.25 pg/ml,18.97±8.78 pg/ml和20.13±10.28 pg/ml,治疗前与恢复期、对照组相比,差异均有统计学意义(t=6.669 1~6.941,P值均〈0.01);患儿组恢复期血清IL-17和IL-22水平与健康对照组相比差异无统计学意义(t=1.290 2,P值〉0.05;t=0.472 7,P值〉0.05).②患儿组治疗前血清IL-17和IL-22水平呈正相关(r=0.593 1,P值=0.000 6〈0.05).结论 RRTI患儿组治疗前血清IL-17 和IL-22 水平与恢复期、健康对照组相比明显升高,且水平高低随病情轻重、病程而变化,提示IL-17 和IL-22参与了RRTI的发病过程,是判断RRTI患儿免疫状态和预后的指标之一.Objective To explore the expression of serum IL-17 and IL-22 levels in children with recurrent respiratory tract infeetions(RRTI) and explore their clinical significances. Methods The levels of IL-17 and IL-22 in the serum of 30 cases of children with RRTI who visited the Affiliated Hospital of Binzhou Medical University between October 2012 and March 2013 in prior treatment and convalescent were determined by enzyme linked immunosorbent assay(ELISA) ,the measurement results were compared with the resultsin 30 cases of healthy control group for the same period. Results ①The serum level of IL-17 of children with RRTI in prior treatment,that in recovery stage of and that in control group were 24. 13±8.80 pg/ ml, 14. 37±8. 22 pg/ml and 12.07±5.27 pg/ml,the serum level IL-17 of children with RRTI in prior treatment were higher than that in control group and recovery stage of RRT1 (t=4. 281±6. 4432, P〈0.01). The serum level of IL-22 of children with RRTI in prior Treatment, that in recovery stage and that in control group were 76.64 ± 45.25 pg/ml, 18. 97-4-8. 78 pg/ ml and 20. 13! 10.28 pg/ml,the serum level IL-22 of children with RRTI in prior treatment were higher than that in recov- ery stages of RRTI and control group (t= 6. 6691±6. 941, P〈0.01). There was no significant difference of the levels of IL- l7 and IL-22 between the convalescence of RRTI and healthy control group (t= 1. 292, P〉0.05;t= 0. 4727, P〉0.05). ② There was positive correlation between IL-17 and IL-22 of children with RRTI in priort reatment (r=0. 5931, P〈0. 05). Conclusion The serum levels of IL-17 and IL-22 of children with RRTI in prior treatment were significantly higher than those of RRTI convalescence and healthy control group. IL-17 and IL-22 participate in RRTI's procedure. The levels change with the pathogenetie condition and the course of disease. Their levels are helpful to determining the immunity condition and pre-after targets.
