摘 要:目的 研究广西地区壮族、瑶族和汉族健康人群中人类血小板抗原(HPA)1~17bw系统基因的差异表达.方法 壮族、瑶族和汉族(祖上三代均为同民族)的健康个体各100例,采用 PCR-SSP方法检测上述3个民族个体的HPA1~17bw系统基因,计算HPA各系统的不配合率和基因多态性,并比较3个民族间HPA系统的基因频率.结果 基因型别a是3个民族HPA1~17bw的主要型别,而3个民族的HPA-3和HPA-15的不配合率均大于30%.比较3个民族血小板系统的基因频率,瑶族人群中的HPA-3基因杂合程度低于壮族(χ2=12.242,P=0.002);瑶族人群中的HPA-15基因杂合程度显著低于壮族(χ2=6.209,P=0.045)和汉族(χ2=12.383,P=0.002),差异有统计学意义.而壮族人群中HPA-3,15基因杂合程度与汉族的差异均有统计学意义(χ2=7.640,P=0.022;χ2=10.685,P=0.005).结论 广西地区瑶族、壮族和汉族HPA基因多态性分布存在明显的种族差异,HPA-3和HPA-15系统在3个民族发生同种免疫反应均有显著意义,在临床血小板输注中应加以重视.Objective To explore the difference of gene polymorphism of human platelet antigen (HPA1 - 17bw) system in Zhuang, Yao and Han nationality of Guangxi region. Methods The genotypes of 17 HPA systems were performed by poly-merase chain reaction using sequence-specific primer (PCR-SSP) for 100 Zhuang individual, 100 Yao individual and 100 Han individual whose ancestor in three generations were the same nationlity,and calculated the mismatch rate of the each HPA gene system and Gene frequency. Results The HPA gene systems of Zhuang,Yao and Han ethnic group mainly showed a/ a homozygous. HPA-1,2,3,5,6,15 all showed homozygous genotype with responsive HPA-b alleles were found. The HPA-3 and HPA-15 mismatch rate of 3 national were greaterthan 30%. Analysis the HPA gene frequency of three national in Guangxi region, the heterozygous degree of HPA-3, gene in Yao nationality were significantly lower than Zhuang (X^2 = 12. 242,P=0. 002). The heterozygous degree of HPA-15 gene in Yao nationality were significantly lower than Zhuang (X^2 = 6. 209, P= 0. 045) and Han nationality (X^2=12. 383, P=0. 002), the difference of heterozygous degree of HPA-3,15 was statistically significant between Zhuang and Han nationality (X^2= 7. 648, P=0. 002; (X^2 = 10. 685, P= 0. 005). Conclusion The ethnic difference was significant in HPA of Zhuang, Yao and Han in guangxi region. HPA-3 and HPA-15 system in three national had the same immune response. Should be attention when platelet transfusion in clinical.
