安心颗粒联合替罗非班对急性心肌梗死介入术后心肌微循环的影响

摘　　要：【目的】观察安心颗粒联合替罗非班对急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)术后冠脉微循环的干预作用。【方法】将120例符合纳入标准的AMI患者随机分为治疗组和对照组各60例,2组均予西医指南规范处理并予静脉使用替罗非班,治疗组加用安心颗粒,观察2组TIMI血流分级、校正的TIMI帧计数(CTFC)、ST段回落、左室功能等的差异。【结果】治疗组TIMI 3级血流比例高于对照组(P<0.01),CTFC低于对照组(P<0.01),ST段回落幅度高于对照组(P<0.05),术后左室射血分数高于对照组(P<0.05),左室舒张末期内径和收缩末期内径有低于对照组的趋势,但差异均无统计学意义(P﹥0.05)。【结论】安心颗粒联合替罗非班可以改善AMI患者PCI术后冠脉微循环障碍,提高患者的心功能。Objective To observe the influence of Auxin Granules Combined with Tirofiban on myocardial microcirculation in acute myocardial infarction （AMI） patients after percutaueous coronary intervention（PCI） . Methods One hundred and twenty qualified AMI patients were equally randomized into treatment group and control group. Both groups were given standard treatment following the 2012 edition of China Guideline for PCI, and were also given intravenous injection of Tirofiban. The treatment group was given oral use of Anxin Granules additionally. The changes of blood flow grading in TIMI （thrombolysis in myocardial infarction trial）, corrected TIMI frame count （CTFC）, ST segment resolution in electrocardiogram （ECG） and left ventricular function were observed in both groups. Results The proportion of TIMI 3 （P〈0.01） and the amplitude of ECG ST segment resolution （P〈0.05） were higher, CTFC was lower （P〈0.01）, and the left ventricular ejection fraction was higher （P〈0.05）in the treatment group than those in the control group significantly. Left ventricular end-diastolic diameter and end systolic diameter of the treatment group showed a decreasing trend, but the differences were insignificant when compared with those in the control group （P〉0.05） . Conclusion Anxin Granules combined with Tirofiba can improve the myocardial microcirculation and heart function of the AMI patients after PCI.
