高血压心电监护拔牙227例观察分析

摘　　要：目的观察分析高血压患者麻醉和拔牙过程中收缩压、舒张压和心率的变化,为此类患者安全施行拔牙手术提供参考。方法高血压患者227例,正常血压患者35例,观察分析麻醉前10 min,麻醉注射即刻,拔牙后即刻、拔牙后3 min收缩压、舒张压和心率的变化,以及患者的主观感觉。结果高血压组手术过程中收缩压升高均值与手术前及正常组比较有显著性差异,舒张压、心率的变化无显著性差异。结论术中严密监测各项心血管参数,重点观察收缩压的升高,高血压患者可安全施行局麻下的拔牙手术。无痛麻醉的使用可降低术中血压的变化程度。Objective To observe the fluctuations of blood pressure and heart rate of the patients with hypertension during tooth extraction. Methods 227 cases with hypertension were in the experiment group and 25 cases were in the control group. The changes of diastolic（DBP）or systolic（SBP）blood pressure, electrocardiogram（EEG）and feelings of the patients were observed before,during and after operation. Results SBP increased significantly during operation （P〈0.01）,there were no significant differences among other indexed. Conclusion Closely monitor the cardiovascular parameters in operation, focusing on elevated systolic blood pressure, hypertension can be performed safely under local anesthesia in extraction operation. The use of painless anesthesia can reduce the change of the intraoperative blood pressure.
