摘  要:目的 探讨不同病期儿童哮喘血清嗜酸细胞阳离子蛋白 (ECP)、肿瘤坏死因子 α(TNF α)和最大呼气流速 (PEF)水平及其相关性和临床意义。方法 对急性发作组、治疗组及缓解组哮喘患儿各 30例和 30例对照组 ,用瑞典PharmaciaCAP试剂盒检测血清ECP ;以放射免疫分析法测定血清TNF α ;峰流速仪测定PEF ,并计算占预计值的百分比 (PEF % )。结果 急性发作组血清ECP明显高于其余3组 (P <0 .0 1) ,其中重症发作者比轻、中症发作者高 (P <0 .0 5 ) ;急性发作组血清TNF α高于缓解组、对照组 (P <0 .0 1) ;PEF %在发作组明显低于其余 3组 (P <0 .0 1) ,重症发作明显低于轻、中症发作 (P <0 .0 1)。PEF %与ECP、TNF α与ECP均呈负相关 (分别为r=- 0 .6 4 P<0 .0 1及r=- 0 .41 P<0 .0 5 ) ;PEF %与TNF α间无明显相关 (r= 0 .0 46 P >0 .0 5 )。结论 血清ECP、TNF α均参与了哮喘发病 ,在评价气道炎症状态中 ,以ECP更敏感 ,PEF测定更方便、更经济 。Objective To study the relationship among eosinophil cationic protein (ECP) 、tumor necrosis factor (TNF α) and peak expiratory flow (PEF) in different phases of children asthma .Methods Four groups were designed, including acute attack group(30 cases)、study group(30 cases)、convalescent group(30 cases) and control group(30 cases).ECP was measured with Pharmacia CAP system and TNF α by radioimmunoassay;PEF was monitored with peak flowmeter and the percentages of predicting value was calculated(PEF %). Results Serum ECP in acute attack group was remarkably higher than that in other 3 groups (P<0.01),the value in severe attack cases was significantly higher than that in light moderate cases(P<0.05); Serum TNF α in acute attack group was higher than that in convalescent group and control group(P<0.01);PEF % in acute attack group was significantly lower than that in other groups(P<0.01), and it was much lower in severe attack than that in light moderate(P<0.01). There was correlation between PEF % and ECP,TNF α and ECP(r=-0.64 P<0.01 and r=-0.41 P<0.05 ) ; No correlation between PEF % and TNF α(r=0.046 P>0.05). Conclusion Serum ECP and TNF α are both involved in asthmatic attack. ECP is more sensitive than others in estimating airway inflammatory states; PEF measurement is more convenient and less costly; both can be effective indications in monitoring patient′s condition and guideing treatment.
