摘  要:目的 探讨结核分枝杆菌rESAT6-CFP10(rEC)和rPstS1-HspX(rPH)融合蛋白用于结核病血清学诊断的价值.方法 以rEC和rPH融合蛋白为抗原,用金标免疫渗滤法(DIGFA)检测370份血清中的抗结核抗体,其中肺结核患者血清131份,非结核肺病患者血清114份,健康人群血清125份.结果 rEC和rPH融合蛋白单一或联合检测肺结核患者血清抗结核抗体的的灵敏度分别为70.23%、71.76%和84.73%.联合检测的灵敏度显著高于rEC或rPH单一检测,且差异有统计学意义(x2=1.181,P<0.01 ;x2=0.036,P<0.05).3种方法用于检测非结核肺病患者和健康人血清的特异性差异均无统计学意义(x2=0.527、0.097,P>0.05).结论 rEC和rPH融合蛋白均有较高的灵敏度和特异性,联合检测有助于进一步提高结核病血清学诊断的灵敏度.Objective To evaluate the serodiagnostic value of recombinant fusion protein rESAT6-CFP10 (rEC) and rPstS1-HspX (rPH) from Mycobacterium tuberculosis.Methods The rEC and rPH were used as antigens to detect tuberculosis antibodies in serum samples from the patients with pulmonary tuberclosis (n=131),patients with non-tuberculosis lung diseases (n=114) and healthy individuals (n=125) by dot immunogold filtration assay (DIGFA).Results The sensitivities of rEC,rPH and combination antigens (rEC and rPH) were 70.23%,71.76% and 84.73%,respectively.The sensitivities of combination antigens was significantly higher than the rEC or rPH alone (x2=1.181,P〈0.01; x2=0.036,P〈0.05).There were no significant differences in specificities among three methods for the serum dection of non-tuberculosis lung disease petients and healthy individuals (x2=0.527,0.097,P〉0.05).Conclusions The rEC and rPH have high sensitivities and specificities in serodiagnosis of tuberculosis and combination detection can improve the sensitivity.
