摘  要:目的 阐明不同脂肪酸构成比对 型糖尿病患者餐后血脂代谢的影响 ,探索具有预防动脉粥样硬化作用的膳食脂肪酸构成比 (SFA∶ MUFA∶ PUFA)。方法 采用脂肪负荷餐后血脂代谢研究法 ,将 2 8例血糖控制稳定的 型糖尿病患者按两级分层抽样被随机地分为三组 ,禁食 1 2 h,分别接受脂肪酸构成比为 1∶ 1∶ 1 ( )、1∶ 1 .7∶ 1 .2 ( )和 1∶ 1 .7∶ 2 .3( )的脂肪负荷餐。测定餐前和餐后 2、4、6h血浆 TG、TC、HDL- C、Apo A1和 Apo B,LDL- C由公式计算得出。结果 各组餐后各时间点 TG较餐前明显升高 ,HDL- C和 Apo A1较餐前降低 , 、 和 组 TG峰时间 2~ 4、4和 4~ 6h,而 HDL- C和 Apo A1峰时间分别为 4、4和 6h;各组餐后 LDL- C和 Apo B较餐前降低 ,峰时间均在 4h。 组 LDL- C餐后 6h浓度和曲线下面积明显低于 和 组。多元逐步回归分析表明 ,MUFA/PUFA是影响餐后 2 h血浆总甘油三酯增值的因素 ,PUFA/SFA是影响LDL- C餐后曲线下面积的重要因素。结论 脂肪酸构成比不同可影响脂肪负荷餐后 型糖尿病患者血脂代谢动态 ; 组脂肪酸构成比可初步作为Objective:[WT5BZ]To explore the effects of different ratios of saturated,monounsaturated and polyunsaturated fatty acids in the oral fat load test on the postprandial lipids and lipoproteins metabolism.[WT5FZ]Methods:[WT5BZ]The oral fat load tests with three different fatty acids ratios(1∶1∶1,1∶1.7∶1.2 and 1∶1.7∶2.3)were carried out in 28 type Ⅱ diabetic patients.There were 9 patients for group Ⅰ,9 for group Ⅱ and 10 for group Ⅲ.The total triglyceride(TG),total cholesterol(TC),cholesterol in high density lipoprotein(HDL C),apoprotein A1(ApoA1)and B(ApoB)in plasma before and 2,4,6 hours after fat load were determined.Cholesterol in low density lipoprotein(LDL C)in plasma was calculated.[WT5FZ]Results:[WT5BZ](1)Lipids concentrations:TG responded greatly to the oral fat load in three groups.The TG in plasma 2,4,6 hours after the loads was significantly higher than fasting TG,respectively(P<0.05).No significant TC changes occured but HDL C and LDL C significantly decreased in group Ⅱ and Ⅲ.(2)Peak time:TG in group Ⅰ peaked at 2~4 hours;in group Ⅱ peaked at 4 hours and in group Ⅲ at 4~6 hours after fat load.HDL C and ApoA1 in group Ⅰ and Ⅱ were lowest at 4 hours after loads but in group Ⅲ were at 6 hours.(3)Area under the curve(AUC):AUC LDL C in group Ⅲ was markedly smaller than that in group Ⅱ.Multiple stepwise regression showed that P/S was the factor that influenced the value of AUC LDL C.[WT5FZ]Conclusion:[WT5BZ]The study shows that fatty acids ratios in the oral fat load test influence the dynamic changes of the postprandial lipids and lipoproteins metabolism in type Ⅱ diabetic patients.
