电话随访在慢性阻塞性肺疾病患者家庭应用噻托溴铵吸入剂中的应用

摘　　要：目的 探讨电话随访对慢性阻塞性肺疾病患者家庭应用噻托溴铵吸入剂方法的正确性、依从性、预防药物不良反应的认知、用药对生活质量的影响,以期达到更好地控制病情,提高患者生活质量的目的.方法 采用随机数字表法将123例使用噻托溴铵的患者分为观察组59例和对照组64例.观察组采用定期电话随访,对照组不做定期电话随访,3个月后比较两组患者用药知识情况和CAT评分.结果 干预前观察组患者CAT评分为（19.10±6.46）分,对照组患者CAT评分为（20.02±6.24）分,两组比较,差异无统计学意义（P＞0.05）.干预3个月后给予定期电话随访的观察组患者CAT评分为（6.98±2.93）分,低于对照组患者的（14.56 ±5.16）分,差异有统计学意义（t=-10.25,P＜0.05）.干预前观察组患者用药指导问卷得分为10分,对照组为10分.干预3个月后观察组患者用药指导问卷评分为（9.73±0.49）分,高于对照组的（4.33±1.26）分,差异有统计学意义（t=32.24,P＜0.05）.结论 电话随访改善了慢性阻塞性肺疾病患者家庭应用噻托溴铵吸入剂的用药情况和患者的生命质量.Objective To discuss the effect of telephone follow-up on COPD patients＇ use of Spiriva,including correctness,compliance,cognition of preventing side effects and Spiriva＇ s influence on their quality of life,so as to better control disease conditions and improve patients＇ quality of life.Methods Totals of 123 patients were randomly divided into the observation group （n =59） and the control group （n =64）.The observation group adopted regular telephone follow-up while the control group did not.Patients＇ knowledge of Spiriva and score of CAT were compared three months later between two groups.Results Score of CAT was （19.10 ± 6.46） in the observation group before intervention and （20.02 ± 6.24） in the control group,with no statistically significant difference （P ＞ 0.05）.Three months after intervention,CAT score was （6.98 ± 2.93） in the observation group and （14.56 ± 5.16） in the control group,and the difference was statistically significant （t =-10.25,P ＜0.05）.Before intervention,the average score of medication guide questionnaire was 10 in both the observation group and the control group.3 months after intervention,the score was （9.73 ±0.49） in the observation group,higher than （4.33 ± 1.26） in the control group,and the difference was statistically significant （t =32.24,P ＜ 0.05）.Conclusions Telephone follow-up can improve COPD patients＇ use of Spiriva and quality of life.
