摘  要:目的研究冠心病不稳定型心绞痛痰浊痹阻证、气虚血瘀证患者的代谢组学特征,探讨代谢组学方法在研究不稳定型心绞痛中医证型中的应用。方法对12例不稳定型心绞痛痰浊痹阻证患者和12例不稳定型心绞痛气虚血瘀证患者的血浆样本进行氢核磁共振(1HNMR)检测。通过偏最小二乘判别分析(PLS-DA)方法研究不稳定型心绞痛痰浊痹阻证患者与气虚血瘀证患者之间血浆代谢产物谱及的差异。结果各组血浆核磁共振氢谱的PCA结果显示,不稳定型心绞痛证痰浊痹阻患者与不稳定型心绞痛气虚血瘀患者能够被区分,2组血浆样本之间的代谢物含量存在明显差异。不稳定型心绞痛痰浊痹阻证组血浆中乳酸、葡萄糖、半乳糖、N-乙酰糖蛋白、低密度脂蛋白、脂类化合物、酮体、天门冬氨酸、谷氨酞胺、酪氨酸、脯氨酸的含量高于气虚血瘀证组,而柠檬酸、高密度脂蛋白、不饱和脂肪酸、氧化三甲胺的含量低于气虚血瘀证组。结论 2组血浆样本的1HNMR代谢谱存在差异,能从代谢组学分析中找出特异的标志性代谢产物。血浆代谢物变化能在一定程度上区分冠心病不稳定型心绞痛的这两种中医证型。Objective To explore the plasma metabonomic profile of unstable angina pectoris patients with phlegm-turbid syndrome(PS) and with Qi-deficieney and blood-stasis syndrome(QBS).To examine potential application of metabonomic analysis in studies of Chinese Traditional Medicine syndromes of Unstable Angina Pectoris patients.Methods Totally 12 cases unstable angina pectoris patients with phlegm-turbid syndrome(PS) and 12 cases of unstable angina pectoris patients with Qi-deficieney and blood-stasis syndrome(QBS) were enrolled in this study and their plasma metabolites were analyzed using Hydro-Nuclear Magnetic Resonance(1H NMR)spectrometer for the Partial Least Squares Discriminant Analysis(PLS-DA).Result PLS-DA of plasma 1H NMR spectra revealed different metabolic spectra between PS patients and QBS patients,demonstrated that the metabolic characteristics of the two groups were significantly different.Compared with QBS patients,lactic acid,glucose,galactose,N-acetyl glycoproteins,low-density lipoprotein,lipid compounds,ketone bodies,aspartate,thalidomide glutamyl,tyrosine,Proline increased in PS patients,while citric?acid,high-density lipoprotein,unsaturated fatty acids,trimethylamine oxide decreased considerably.Conclusion Characteristic metabolic products in two groups can be identified by 1HNMR based metabonomics analysis.Metabonomic study is a feasible and promising way to detect the variation of plasma etabonomics ofUnstable Angina Pectoris patients with differentTCMSyndromes.
