摘  要:目的调查广东中山小榄地区孕早期孕妇TORCH(巨细胞病毒、风疹病毒、弓形虫和单纯疱疹病毒总称)-IgM阳性率,为优生优育提供参考依据。方法选取2011—2012年来本院行孕期检查的孕早期孕妇4 020例,采用微粒子酶联免疫分析法检测巨细胞病毒-IgM抗体(CMV-IgM)、风疹病毒-IgM抗体(RV-IgM)和弓形虫-IgM抗体(TOXO-IgM),采用酶联免疫吸附法(ELISA)检测单纯疱疹病毒(Ⅰ+Ⅱ)-IgM抗体(HSV-IgM),计算其阳性率,并对结果进行分析。结果孕早期TORCH-IgM的总阳性率为7.11%(286/4 020),其中CMV-IgM的阳性率最高(5.37%,216/4 020),与RV-IgM(0.70%,28/4 020)、TOXO-IgM(0.45%,18/4 020)和HSV-IgM(0.60%,24/4 020)阳性率比较,差异均有统计学意义(P=0.000),而后三者阳性率间差异无统计学意义(P>0.05)。孕早期TORCH-IgM阳性率在不同地区间差异有统计学意义(P<0.05);而在不同年龄组间差异无统计学意义(P>0.05)。结论小榄地区孕早期孕妇以CMV-IgM阳性率最高,且高于其他三者之和;TROCH各病原体的感染率在各年龄阶段分布是稳定的,不随年龄的变化有所改变。因此,任何年龄孕早期的孕妇均应进行TORCH筛查,尤其是CMV感染的检查,这对优生优育十分重要。Objective To investigate the positive rates of TORCH - IgM ( including Cytomegalo virus, Rubella virus, Toxoplasma gondii and Herpes simplex virus) in early pregnant women in Xiaolan district to provide a basis for prepotency. Methods Cytomegalo - IgM antibody ( CMV - IgM ), Rubella - IgM antibody ( RV - IgM ), toxoplasma gondii - IgM (TOXO- IgM ) were detected by microparticle enzyme immunoassay, herpes simplex virus ( Ⅰ + Ⅱ ) -IgM antibody ( HSV - IgM) by enzyme - linked immunosorbent assay (ELISA) in 4 020 prenatal trimester pregnant women having pregnancy checks in this hospital from 2011 to 2012. The positive rates were calculated and results analyzed. Results The total positive rate of early pregnancy TORCH -IgM was 7.11% (286/4 020), thereinto CMV -IgM positive rate was the highest (5.37%, 216/4 020), significantly different from positive rates of RV - IgM ( 0. 70%, 28/4 020 ), TOXO - IgM ( 0.45%, 18/4 020 ), HSV - IgM (0. 60%, 24/4 020) ( P = 0. 000), and there was no difference in positive rate between the latter 3 ( P 〉 0.05 ). There was difference in TORCH - IgM positive rate between different regions ( P 〈 0. 05 ), but there was no between dif- ferent age groups ( P 〉 0. 05 ). Conclusion CMV - IgM positive rate is the highest, higher than the sum of the other 3. The infection rates of TORCH pathogens are stable in distribution of all ages, not changing with the changes of age. Therefore, early pregnant women of any age should have TORCH screening, especially CMV infection screening, which is very important for pre- natal and postnatal care.
